超声联合64层CT诊 断复杂先天性心脏 病的临床研究

摘要

目的 探析超声联合64层CT诊 断复杂先天性心脏病的临床价值以期 提高该病诊断准确率。方法 选择我院 2013年3月-2015年3月收治62例复杂性 心脏病患者作为研究对象,所有患者均 于术前采取超声及64层CT检查,以手术 证实结果为基准,将超声、64层CT及联 合检查诊断准确性进行对比。结果 62 例患者经手术证实,各种心脏畸形共计 158处,节段分析法得到:心内与房室 连接处畸形共82处、心脏大血管连接处 畸形共48处、心外大血管畸形共28处。 超声诊断准确率为91.8%(145/158)、CT 检查为91.1%(144/158),联合检查为 97.5%(154/158),联合确诊率明显更 高,与超声及CT的的比较均有统计学意 义(P<0.05)。结论 超声联合64层CT诊 断复杂先天性心脏病可获取患者心脏缺 损及异常通道部位、畸形血管走行方 向、冠状动脉与体肺侧支血管异位开口 等信息,让疾病情况得到良好显示,诊 断准确性较高,临床借鉴价值大。

Objective To explore the clinical value of ultrasound combined with 64- slice CT in the diagnosis of complex congenital heart disease to improve the accuracy of diagnosis. Methods 62 patients who had complex heart disease and were admitted into the hospital during March 2013 to March 2015 were selected as the research objects. All patients received ultrasound and 64-slice CT examination before surgery. Based on the results confirmed by surgery, the diagnostic accuracy of ultrasound, 64-slice CT and the combination of the two was compared. Results Confirmed by surgery, there were a total of 158 cardiac malformations in the 62 cases of patients. Sequential segmental analysis showed that there were 82 malformations in the junction of intra-heart and atrioventricular, 48 malformations in the junction of cardiac large vessels and 28 malformations in extracardiac large vessels. The diagnostic accuracy rate of ultrasound was 91.8% (145/158) while of CT was 91.1% (144/158) and of combined detection was 97.5% (154/158), which showws that the diagnosed rate of the combined detection was significantly higher. The difference was statistically significant (P<0.05). Conclusion Ultrasound combined with 64-slice CT in the diagnosis of complex congenital heart disease can obtain such information as heart defects and sites of abnormal passage, the direction of the abnormal blood vessels, the ectopic opening of coronary artery and pulmonary coronary collateral vessels. It can well show the condition of disease, with relatively higher accuracy, which is of great clinical reference value.

前言

先天性心脏病指胎儿时期心脏血管发育即出现异常或出生后应关 闭通道未能顺利闭合所引起局部解剖结构异常,严重威胁患者健康与 生命安全[1]。复杂先天性心脏病以手术矫正为主要治疗方法,有效诊 断方法极为重要,且术前需进行相关检查以全面了解畸形特征并据此 对手术风险进行评估[2]。超声在复杂先天性心脏病临床诊断中的应用 有一定局限,因此目前多采取联合检查确诊以提高诊断准确性。本文 以我院收治复杂性心脏病患者为例,对超声联合64层CT诊断复杂先天 性心脏病的临床价值进行探析,现报道如下。
