多层螺旋CT多平面重组对泪道阻塞的术前诊断价值分析*

摘要

目的 多层螺旋CT(MSCT)多平面 重组(MPR)对泪道阻塞的术前诊断价值分析。方法 选择2016年5月到2018年5月到医院就诊泪道阻塞患者80例,其中15例患者为骨性阻塞者。所有患者均进行MSCT扫描,并采用MPR技术进行后处理。检测所有患者不同方位健侧及患侧泪囊面积,统计不同阻塞部位的CT检出符合率,检测骨性阻塞患者健侧及患侧的骨性鼻泪管的相关指标。结果 患者患侧泪囊面积在轴位、斜矢状位、斜冠状位均大于健侧 (P<0.05);以内镜为金标准,CT检查对鼻泪管上段、中段、下段及泪囊鼻泪管结合部位的检出符合率分别为96.49%、 85.71%、83.33%及100.00%;骨性阻塞患者患侧垂直径及横径的起始段、中段、末段及高处数据均小于健侧(P<0.05)。结论 MSCT的MPR技术检测泪道阻塞对不同阻塞部位检出符合率较高,利于术后手术方案选择。

Objective To analyze the preoperative diagnostic value of multi-slice spiral  computed tomography (MSCT) multiplanar recombination (MPR) for lacrimal duct  obstruction. Methods 80 patients with lacrimal duct obstruction who were treated in the  hospital from May 2016 to May 2018 were selected and 15 of them were bony obstructive.  All patients were given MSCT scans and post-processing with MPR technique. The  lacrimal sac areas of patients with different orientation of unaffected side and affected  side were detected. The CT detection coincidence rate of different obstructive sites  was counted, and the relevant indexes of the bony nasolacrimal duct were detected in  the unaffected side and the affected side of patients with bony obstruction. Results The  lacrimal sac areas in axial, oblique sagittal and oblique coronal positions of affected side  were larger than those of unaffected side (P<0.05). The endoscopy was taken as gold  standard, and the detection coincidence rates of CT examination on the upper, middle  and lower segments of nasolacrimal duct and junction of lacrimal sac and nasolacrimal duct  were 96.49%, 85.71%, 83.33% and 100.00% respectively. The initial, middle, terminal,  and elevation data of vertical diameter and transverse diameter of affected side of patients  with bony obstruction were less than those of unaffected side (P<0.05). Conclusion MPR  technique of MSCT has high detection coincidence rate of different obstruction sites in  the detection of lacrimal duct obstruction, and it is beneficial to the choice of postoperative  surgical regimen.

前言

  泪道阻塞是一种眼科常见疾病,常发生部位有泪小管、泪点、鼻 泪管下口及泪囊与鼻泪管交界,临床主要表现为溢泪[1-3]。临床上多采用泪道冲洗及泪道探针进行检测,但泪道冲洗无客观评价依据,泪道探针又可能导致额外损伤,有一定局限性。电子计算机断层扫描 (computed tomography,CT)检查有较高的密度及空间分辨率,图像清晰,可明确显示解剖关系[4];其后处理技术多平面重组(multiplane recombination,MPR)指将扫描范围内轴位图像叠加并进行重组得到图像,可更好显示弯曲物体信息[5]。基于此,本研究采用多层螺旋 CT(multi-slice spiral CT,MSCT)的MPR技术检测泪道阻塞,以期探究其检测效果。
