摘要
目的 探讨磁共振成像(MRI)在血管性认知障碍(VCI)患者功能改变中的诊断效果。方法 纳入我院收治的VCI患者98 例,将其作为研究组,并选取同期于我院进行体检的98例健康者作为对照组,利用认知功能量表对VCI患者予以评 估,并比较两组MRI检查结果。结果 ①痴呆型VCI患者CCSE与MMSE评分较非痴呆型VCI患者更高,枕叶、顶叶、额 叶、颞叶萎缩程度大于非痴呆型VCI患者,差异有统计学意义(P<0.05);②研究组枕叶、顶叶、额叶、颞叶萎缩程 度高于对照组,两组对比有差异(P<0.05)。结论 磁共振影像能反映血管性认知障碍患者的功能改变情况,临床可 将MRI与认知功能评分联合应用于VCI诊断中,提高诊断准确率。
Objective To investigate the diagnostic value of magnetic resonance imaging (MRI) on functional changes in patients with vascular cognitive impairment (VCI). Methods 98 VCI patients treated in our hospital were selected as the research group, while another 98 healthy people examined in our hospital were selected as the control group. All the VCI patients were evaluated by the cognitive function scale, and the results of MRI examinations were compared between groups. Results The scores of CCSE and MMSE in patients with dementia VCI were higher than those in nondementia VCI patients, the atrophy degrees of occipital, parietal, frontal and temporal lobes in patients with dementia VCI were greater than those in non-dementia VCI patients, and their differences were statistically significant (P<0.05). The atrophy degrees of occipital, parietal, frontal and temporal lobes in the research group were greater than those in the control group, and their differences were statistically significant (P<0.05). Conclusion MRI can reflect the functional changes of patients with vascular cognitive impairment. Therefore, MRI combined with cognitive function scale can be used in the diagnosis of VCI, so as to improve the accuracy of diagnosis.

前言
血 管 性 认 知 障 碍 ( v a s c u l a r c o g n i t i v e impairment,VCI)发病机制比较复杂,其发生与高血 脂、糖尿病等脑血管病危险因素密切相关[1]。研究 表明,在年龄超过65岁老年群体中,VCI患病率约 2.6%[2]。认知功能评分为VCI诊断提供了依据,现阶 段,临床在该病诊断时所应用的评分量表较多,包括 认知能力筛查量表(cognitive capacity screening e x a m i n a t i o n , C C S E )、简易智能状态量表 ( m i n i mental state examination,MMSE)等,不过量表评分 仅为量化标准,并不能明确患者功能改变情况,因 此,临床需借助影像学手段观察患者病症变化。磁共 振成像(magnetic resonance imaging,MRI)在多种疾病诊断中应用广泛,它能通过三维断面成像为疾病诊 断提供信息,可清晰显示受检部位软组织结构,诊断 效果显著。目前关于MRI诊断VCI患者功能改变的相 关报道减少,鉴于此,本院选取98例VCI患者进行研 究,分析MRI在VCI功能改变中的诊断价值,现报道如 下。
