高压氧对重型颅脑损伤患者认知功能超敏C反应蛋白水平的影响

目的 探究高压氧对重型颅脑损伤患者认知功能和超敏C反应蛋白(hs-CRP)水平的影响。方法 将我院2016年3 月-2017年3月收治120例重型颅脑损伤患者按照治疗方式不同分为观察组(n=60)和对照组(n=60),对照组采用常规 治疗,观察组采用常规治疗辅助高压氧治疗,比较两组治疗疗效、治疗前后HDS评分、CDR评分和血清hs-CRP水平。 结果 观察组疗效总有效率(96.67%)显著高于对照组(80.00%),差异具有统计学意义(P<0.05);观察组治疗后HDS评 分(23.51±4.52)显著高于对照组(16.59±4.22),CDR评分(0.52±0.41)显著低于对照组(1.15±0.68),差异具有 统计学意义(P<0.05);观察组治疗后hs-CRP水平(1.77±1.07)显著低于对照组(5.92±1.90),差异具有统计学意义 (P<0.05)。结论 高压氧辅助治疗重型颅脑损伤可以显著改善患者认知功能和炎症反应,值得临床推广。
Objective To investigate the effect of hyperbaric oxygen on cognitive function and high sensitivity c-reactive protein (hs-CRP) level in patients with severe craniocerebral injury. Methods From Mar. 2016 to Mar. 2017, 120 cases of severe craniocerebral injury patients in our hospital were divided into observation group (n=60) and control group (n=60) according to their treatment methods. Both groups were given routine treatment, besides, the observation group was given additional hyperbaric oxygen. The therapeutic effects, HDS scores, CDR scores and serum hsCRP levels before and after treatment in the two groups were compared. Results The total effective rate in the observation group was (96.67%) which was significantly higher than that in the control group (80%), the difference was statistically significant (P<0.05), after treatment, the HDS score in the observation group was (23.51±4.52) which was significantly higher than that in the control group (16.59±4.22), and the CDR score in the observation group was (0.52±0.41) which was significantly lower than that in the control group (1.15±0.68), the difference was statistically significant (P<0.05), the hs-CRP level in the observation group after treatment was (1.77±1.07) which was significantly lower than that in the control group(5.92±1.90), the difference was statistically significant (P<0.05). Conclusion The adjuvant therapy of hyperbaric oxygen can significantly improve the cognitive function and inflammatory response of patients with severe craniocerebral injury, thus, it is worthy of clinical promotion.

前言
颅脑损伤作为临床上一种常见创伤外科疾病, 致残率高,重型颅脑损伤是指由于强外力作用导致 患者在伤后昏迷时间超过12h,伴随有明显神经系统 阳性体征的急危重症,治疗困难,死亡率高达30% 以上[1-2]。高压氧作为具有特殊疗效的氧气,可以缓 解颅脑损伤患者的脑水肿现象,降低颅内压,改善 脑缺氧,促进能量代谢,提高术后治愈率,降低患者死亡率[3]。本文旨在探究高压氧对重型颅脑损伤 患者认知功能和超敏C反应蛋白(high-sensitivity C-reactive protein, hs-CRP)水平的影响,现报道 如下。
