血清ACE2结合冠状 动脉CT血管成像对 冠心病的诊断价值

目的 观察血清血管紧张素转化 酶2(ACE2)结合冠状动脉CT血管成像(CTA) 对冠心病(CHD)的诊断价值。方法 对116 例拟诊为CHD患者的临床资料进行回顾性 分析。患者均接受冠脉CTA检查,检测血 清ACE2水平,并在3个月内接受冠状动脉 造影(CAG)检查。以CAG检查为金标准,比 较冠脉CTA、ACE2及二者联合诊断CHD的诊 断效能差异。结果 血清ACE2联合冠脉CTA 诊断CHD准确度、灵敏度、特异度分别为 96.55%、97.65%、93.55%,高于冠脉CTA 单独诊断,但比较差异无统计学意义(P> 0.05);血清ACE2联合冠脉CTA诊断Kappa 值0.912大于冠脉CTA单独诊断的0.851。 结论 血清ACE2联合冠脉CTA诊断CHD较冠 脉CTA单独诊断更有助于提升诊断准确度 和灵敏度,与CAG一致性较高。
Objective To observe the diagnostic value of serum angiotensin-converting enzyme 2 (ACE2) combined with coronary CT angiography (CTA) for coronary heart disease (CHD). Methods The clinical data of 116 patients with CHD were analyzed retrospectively. All patients were given coronary CTA, and serum ACE2 level was measured, and they were given coronary arteriography (CAG) within 3 months. CAG examination was used as the gold standard to compare the diagnostic efficacy of coronary CTA, ACE2 and combined diagnosis for CHD. Results The accuracy, sensitivity and specificity of serum ACE2 combined with coronary CTA in the diagnosis of CHD were 96.55%, 97.65% and 93.55% respectively, which were higher than those of coronary CTA alone (P>0.05). Kappa value of serum ACE2 combined with coronary CTA was greater than that of coronary CTA alone (0.912 vs 0.851). Conclusion Serum ACE2 combined with coronary CTA can improve the accuracy and sensitivity in the diagnosis of CHD compared with coronary CTA alone, and it has a high consistency with CAG.

前言
冠状动脉动脉粥样硬化或血管痉挛将导致冠脉狭窄或阻塞,引起 冠脉循环障碍,进而导致心肌缺血缺氧或坏死,即为冠心病(CHD)[1]。 随我国人口老龄化加剧,人们生活水平提高,近年CHD发病率呈上升 趋势。早期诊断和治疗对疾病预后有重要意义。目前,冠状动脉造影 (CAG)仍是国内外专家学者公认诊断CHD的金标准,但其存在为有创、 可重复性差等缺点[2]。随着CT技术的发展,冠脉CT血管成像(CTA)作为 一种具备扫描速度快、分辨率高、无创等特点的检查方式,在心血管 疾病诊断中发挥了重要作用[3]。血管紧张素转化酶2(ACE2)则被证实与 CHD发生、发展密切相关,联合CTA可能对提升诊断效果有一定帮助[4]。 对此,本研究观察血清ACE2联合冠脉CTA在CHD诊断中的应用情况,现 报告如下。
