前交叉韧带损伤的MRI与超声诊断效能比较的临床研究

摘要

目的 对比分析磁共振成像(MRI) 与超声对膝关节前交叉韧带(ACL)损伤 的诊断效能。方法 回顾性收集2015年6 月-2017年6月我院骨科疑似ACL损伤患者 110例,术前均行MRI、超声检查,并将影 像结果与关节镜结果进行对比。结果 本 组共110例患者,经关节镜证实,ACL完全 损伤63例,部分损伤28例,5例存在吸收 或萎缩,正常14例。以关节镜结果诊断结 果为“金标准”,MRI诊断ACL损伤的准 确性、敏感性、特异性、阴性预测值、 阳性预测值、kappa值分别为90.00%、 92.71%、71.43%、95.70%、58.82%、 0.612,超声诊断ACL损伤的准确性、敏 感性、特异性、阴性预测值、阳性预测 值、kappa值分别为84.55%、88.54%、 57.14%、93.41%、42.11%、0.586,组间 比较差异无统计学意义(P>0.05)。结论 MRI与超声对ACL损伤均有较高诊断价值, 超声虽诊断效能较低,但具有操作简便、 检查时间短、费用低等优势,在临床具体 运用中可依据各自优缺点进行选择。

Objective To analyze the diagnostic efficiencies of magnetic resonance imaging (MRI) and ultrasound for knee anterior cruciate ligament (ACL) injury comparatively. Methods A total of 110 patients with suspected ACL injury treated in the department of orthopedics of the hospital from June 2015 to June 2017 were collected retrospectively. All patients underwent MRI and ultrasonography before operation, and the imaging results were compared with arthroscopic findings. Results Among the 110 cases, 63 cases were ACL complete injury, 28 cases were partial injury, 5 cases of absorption or atrophy and 14 cases were normal confirmed by arthroscopy. With the arthroscopic results as the golden standard, The diagnostic accuracy, sensitivity, specificity, negative predictive value, positive predictive value and kappa value of MRI for ACL injury were 90.00%, 92.71%, 71.43%, 95.70%, 58.82% and 0.612, respectively, while those of ultrasound were 84.55%, 88.54%, 57.14%, 93.41%, 42.11% and 0.586, respectively (P>0.05). Conclusion Both of MRI and ultrasound are of high diagnostic value for ACL injury. Ultrasound has advantages of noninvasive, low cost and short examination time, and we can choose a suitable examination method according to the advantages and disadvantages of each examination method in clinical practice.

前言

前交叉韧带(anterior cruciate ligament, ACL)损伤是膝关节损 伤常见类型之一,可引起膝关节肿痛及运动障碍,若不能及时治疗, 可致膝关节不稳,甚至并发软骨、半月板等膝关节其他结构损伤,造 成骨性关节炎,严重危害膝关节健康[1]。因此,ACL损伤的早期诊断对 改善预后尤为重要。目前,临床诊断ACL损伤的方法主要包括病史询 问、体格检查、磁共振成像(magnetic resonance imaging,MRI)、关 节镜检查等,但这些检查手段均存在一定局限性。随着超声设备、技 术不断发展,基于其简便、快捷、经济、可重复性高等特点,超声在 ACL损伤诊断中作用逐渐引起临床重视[2]。本研究回顾性分析110例行 MRI、超声及关节镜检查的ACL损伤疑诊患者临床资料,旨在探讨MRI与 超声对ACL损伤的诊断价值,现报告如下。
