DWI单指数模型和 IVIM模型参数在前 列腺癌诊断中的价 值

目的 探讨MR扩散加权成像 (DWI)单指数模型和体素内不相干运动 (IVIM)模型参数在前列腺癌诊断中的价 值。方法 选取2016年1月至2018年6月 在我院治疗的前列腺癌患者55例,共计 72个病灶,同时选取前列腺增生患者50 例,共计66个病灶作为对照,均给予 DWI扫描,测量单指数模型参数[表观弥 散系数(ADC)]以及IVIM模型参数[慢速 扩散系数(D)、快速扩散系数(D*)、快 速扩散所占容积分数(f)]。结果 前列 腺癌ADC值为(0.723±0.122)×10-3mm2 / s,明显低于前列腺增生,差异比较有 统计学意义(P<0.05);前列腺癌D值为 (0.702±0.101)×10-3mm2 /s,明显低于前 列腺增生,差异比较有统计学意义(P> 0.05);前列腺癌和前列腺增生D*和f比 较差异无统计学意义(P>0.05);ADC和 D诊断前列腺癌的ROC曲线下面积分别为 0.800和0.854,P<0.05,截断值分别为 0.811×10-3mm2 /s和0.802×10-3mm2 /s,灵 敏度分别为82.00%和85.50%,特异度分别 为75.00%和83.00%。结论 DWI单指数模型 参数ADC、IVIM模型参数D在前列腺癌诊断 中有较好的应用价值,值得临床使用。
Objective To investigate the diagnostic value of MR diffusion weighted imaging (DWI) single index model and incoherent voxel intra movement (IVIM) model parameters in prostate cancer. Methods A total of 55 cases of prostate cancer in our hospital from January 2016 to June 2018 were selected, with a total of 72 lesions, and 50 cases of prostatic hyperplasia were selected, with a total of 66 lesions were taken as the control, all of them were given DWI scan, the parameters of the single exponential model [apparent diffusion coefficient (ADC)] and IVIM model parameters [slow diffusion coefficient (D), fast diffusion coefficient (D*), and volume fraction (f)of rapid diffusion]were measured. Results The ADC value of prostate cancer was (0.723±0.122)×10-3mm2 /s, which was significantly lower than that of prostatic hyperplasia, and the difference was statistically significant (P<0.05). the D value of prostate cancer was (0.702±0.101)×10-3mm2 /s, which was significantly lower than that of prostatic hyperplasia, and the difference was statistically significant (P>0.05). There were no significant difference in D* and f between prostate cancer and prostatic hyperplasia (P>0.05). The area of ROC of ADC and D in the diagnosis of prostate cancer were 0.800 and 0.854, P<0.05, the cut-off values were 0.811×10-3mm2 /s and 0.802×10-3mm2 /s, the sensitivity were 82.00% and 85.50%, and the specificity were 75.00% and 83.00%. Conclusion DWI single exponential model parameters ADC and IVIM model parameters D have a good application value in the diagnosis of prostate cancer, which is worthy of clinical application.

前言
前列腺癌(Prostate cancer,PCA) 在老年男性中发病较普遍,在 国外PCA因具有高发病率、高死亡率深受人们重视[1]。2014年美国癌症 统计显示[2],被检查出的PCA患者近25万例,约为男性恶性肿瘤总数的 26%。在我国社会快速发展的今天由于生活作息的紊乱和环境因素的影 响,男性PCA发病率越来越高。目前,因不相干运动(IVIM)和磁共振扩 散加权成像(DWI) 两种技术在检测PCA方面具有突出优势,现阶段受 到了影像医师的青睐[3]。DWI单指数模型参数较其他模型参数有突出的 灵敏度及准确性[4];而IVIM作为新的功能MRI技术,主要用来检测分离 扩散与灌注成分,辨别组织内单纯水分子运动的真性扩散和微循环血 液灌注形成的假性扩散,进而为疾病治疗提供准确的信息[5]。本研究 以PCA患者与前列腺增生患者作为研究对象,均测量DWI参数以及IVIM 模型参数,探讨两种参数在治疗PCA中的作用,为临床应用提供科学依 据。
