双源CT双能量成像技术在骨关节金属内固定去伪影成像的应用

摘要

目的探讨双源CT双能量成像 技术在骨关节金属内固定去伪影成像的 临床应用价值。方法 收集我院2015年4 月-2017年10月我院收治的进行双源CT双 能量扫描骨关节外伤金属内固定术后复 查的37例患者的影像学资料。把所得的 80kV及140kV数据进行线性融合成像,所 得图像为观察组,对图像进行模拟常规 120 kV重建,所得图像为对照组,对照组 和观察组均进行多平面重建技术(MPR)、 容积显示(VR)和最大密度投影(MIP), 并观察重建后对图像质量及去伪影的临 床应用价值。结果 观察组得到的图像 质量为优者平均达91.89%(102/111), 对照组图像质量为优者平均达 43.24%(48/111)。观察组优于对照组, 两组间图像质量差异具有统计学意义 (P<0.05);观察组得到的图像中几乎无 伪影的所占比为92.79%(103/111),对照 组所得到的图像中几乎无伪影所占比为 36.94%(41/111)。观察组优于对照组, 两组间所产生的伪影差异具有统计学意 义(P<0.05)。结论 双源CT双能量成像技 术在骨关节金属内固定中能有效去除金 属伪影,能清楚显示骨关节金属内固定 的具体情况,且对周围肌肉组织显示清 楚,为临床提供了影像学资料具有重要 指导作用。

Objective To investigate the clinical value of dual-source CT dual-energy imaging in de-artifact imaging of bone and joint metal. Methods The imaging data of 37 patients who underwent dual-source CT dual-energy scanning post-metal fixation of bone and joint trauma were collected from April 2015 to October 2017 in our hospital. The resulting 80kV and 140kV data were linearly fused and the resulting images were taken as the observation group. The images were simulated by conventional 120 kV reconstruction and the obtained images were control group. Both MPR, VR and MIP were performed in the control group and the observation group. The clinical value of reconstructed image quality and artifact was also observed. Results The image quality of the observation group was 91.89% (102/111) on average, while that of the control group was on average 43.24% (48/111). The observation group was superior to the control group, with significant difference in image quality between the two groups (P<0.05). In the observation group, there was almost no artifacts in the images, accounting for 92.79% (103/111). There was almost no artifacts in the control group (36.94%, 41/111). The observation group was superior to the control group, and the difference of artifacts produced by the two groups was statistically significant (P<0.05). Conclution Dual-source CT dual-energy imaging can effectively remove metal artifacts in the internal fixation of bone and joint, and can clearly show the internal fixation of bone and joint. And the display of bilateral psoas muscle is very clear, this technology provides clinical imaging information, it has an important guiding role.

前言

内固定术是指在断骨内或从断骨外将断骨用金属螺钉、钢板、髓 内针、钢丝或骨板等物直接连接固定起来的手术[1-2]。骨折切开复位 术及切骨术多采用这种手术,内固定术能较好的保持折端的复位[3]。 但是随着的临床应用的推广,其不足之处逐渐显现出来,由于内固定 术多采用金属置入患者体内,因此,其复查受限,不能进行核磁共振 (MRI)检查,而CT检查容易产生伪影,如果伪影明显,将会严重影响到 对邻近重要结构的诊断[4]。本文为此进行了研究探讨,旨在针对双源 CT双能量成像技术在骨关节金属内固定去伪影成像的临床应用价值。
