MRI联合超声在诊断卵巢癌中的应用

摘要

目的探讨磁共振(MRI)联合超 声(US)在诊断卵巢癌中的应用价值。方 法 选取2015年1月-2017年8月医院妇科收 治的78例卵巢癌患者为研究对象,均行盆 腔MRI、US等影像学检查,以手术病理组 织确诊,明确病理组织类型及临床分期, 并以此为标准,分析MRI联合US诊断卵巢 癌的效果。结果 以手术病理为金标准, MRI联合US诊断卵巢癌的病理类型准确率 为91.03%,明显高于MRI、US单独诊断的 64.10%、60.27%,差异有统计学意义(P <0.05)。MRI联合US诊断卵巢癌的临床分 期准确率为93.59%,明显高于MRI、US单 独诊断的73.08%、71.79%,差异有统计学 意义(P<0.05)。结论 MRI联合US可有效 提高卵巢癌的诊断准确率,对病理类型及 临床分期有突出的鉴别效果。

Objective To investigate the value of magnetic resonance imaging (MRI) combined with ultrasound (US) in the diagnosis of ovarian cancer. Methods 78 patients with ovarian cancer who were admitted to the department of gynecological oncology of the hospital from January 2015 to August 2017 were selected as subjects. All patients underwent MRI and US examinations. The pathological type and clinical stages were made clear, and effects of MRI combined with US in the diagnosis of ovarian cancer were analyzed. Results The accuracy rate of MRI combined with US in the diagnosis of the pathological type of ovarian cancer (91.03%) was significantly higher than that of MRI and US (64.10% and 60.27%) with surgery and pathology as the golden standard (P<0.05). The accuracy rate of MRI combined with US in the diagnosis of clinical stage of ovarian cancer (93.59%) was also significantly higher than that of MRI and US (73.08% and 71.79%) (P<0.05). Conclusion MRI combined with US can effectively improve the diagnostic accuracy of ovarian cancer, with prominent effects in the differential diagnosis of pathological type and clinical stage.

前言

卵巢上皮性癌是女性最常见的恶性肿瘤之一,其致死率在妇科肿 瘤中居首位,发病率仅次于子宫颈癌和子宫体癌,其具有种类繁多, 发病率高,组织成分复杂,预后差等特点。随着生活节奏的加快,人 们生活方式与习惯的改变及工作生活压力增大,加之卵巢癌筛查技术 普及提高、女性吸烟率增加等因素,卵巢癌的发病呈逐渐年轻化趋势 [1-2]。及早诊断与制定治疗方案对改善患者生存质量及延长生存期至关 重要。但在临床实际中,由于卵巢癌患者早期无典型症状,多数一经 查出即为肿瘤中晚期,预后较差,研究[3]显示,中晚期卵巢癌患者的5 年生存率仅为30%~35%。近年来,随着科学技术与影像学的发展,磁 共振(magnetic resonance,MRI)、超声(ultrasound,US)的应用越 来越普遍,前者图像质量稳定、清晰,广泛应用于盆腔疾病的诊断; 后者重复性高,具有简便快捷性、价格低廉性、可移动性等特点。为 此,本研究进一步分析MRI联合US在卵巢癌中的诊断应用价值。
