摘要
目的分析MRI多序列扫描诊断 乳腺癌的临床价值。方法 收集2013年1 月-2017年4月于我院进行乳腺MRI检查的 150例乳腺肿瘤患者的临床资料,所有患 者入院后均接受1.5T MRI常规平扫、动 态增强成像(DCE)、扩散加权成像(DWI) 检查,分析MRI多序列扫描诊断乳腺癌的 临床价值。结果 MRI不同序列诊断乳腺 癌以DWI序列诊断效能最高,敏感性、特 异性及准确率分别为93.40%、75.00%、 88.00%,其次为DCE扫描,敏感性、特异 性及准确率为86.79%、61.36%、79.33%; MRI平扫综合价值最低,敏感性、特异性 及准确率为73.58%、50.00%、66.67%。结 论 MRI DWI序列诊断乳腺癌综合价值最 高,其次为MRI动态增强扫描。
Objective To analyze the clinical value of MRI multi-sequence scan in the diagnosis of breast cancer. Methods The clinical data of 150 patients with breast cancer examined by MRI in the hospital from January 2013 to April 2017 were collected. All the patients underwent 1.5T conventional MRI scan, dynamic contrast-enhanced imaging (DCE) and diffusion weighted imaging (DWI) examination. The clinical value of MRI multi-sequence scan in the diagnosis of breast cancer was analyzed. Results The diagnostic efficiency of DWI sequence in different MRI sequences was the highest, and the sensitivity, specificity and accuracy were 93.40%, 75.00% and 88.00%, respectively, followed by DCE scan, and the sensitivity, specificity and accuracy were 86.79%, 61.36% and 79.33%, respectively. The synthetic value of MRI plain scan was the lowest, and the sensitivity, specificity and accuracy were 73.58%, 50.00% and 66.67%, respectively. Conclusion The comprehensive value of MRI DWI sequence is the highest in the diagnosis of breast cancer, followed by MRI dynamic contrast- enhanced scan.

前言
乳腺癌是女性常见、多发恶性肿瘤之一,近年来,随人们对乳腺 疾病重视程度的加深及常规体检筛查的普及,乳腺癌检出率逐渐上 升,且趋向年轻化[1]。报道发现,不同病理特征乳腺癌患者预后不尽 相同,其中晚期浸润性乳腺癌5年生存率低于70%[2]。而早期确诊及治 疗乳腺癌有助于改善患者预后,提高其生存率。目前影像学检查是乳 腺癌辅助诊断的主要手段,常规检查方式包括乳腺钼靶X线、超声、CT 及乳腺MRI等,其中钼靶X线对乳腺细小钙化敏感度较高,但对腺体深 部病变及致密性乳腺诊断价值有限;而乳腺超声操作简单,无辐射, 但分辨率有限,且受临床经验影响较大;CT筛查其辐射剂量大,且空 间分辨率低,对囊实性病变诊断价值有限,同时其软组织分辨率不及 MRI[3]。而MRI则有较好的软组织分辨率,实现多角度、多平面成像, 配合多序列扫描可清晰显示病变范围、病变类型及生长特点,极大提 高了乳腺病变检出率[4]。为进一步证实MRI多序列扫描诊断乳腺癌的临 床价值,现对医院收治的150例乳腺肿瘤患者的临床资料进行了回顾性 分析,整理报道如下。
