MRA与DSA在颅内动 脉瘤诊断的比较研 究

摘要

目的 比较磁共振血管造影(MRA) 与数字减影脑血管造影(DSA)在颅内动脉 瘤(IAN)诊断中的应用价值。方法 回顾性 分析我院收治且经手术病理确诊的52例 IAN患者的临床资料,所有患者入院后均 接受MRA、DSA检查,比较MRA、DSA诊断颅 内动脉瘤的准确性,分析其诊断价值。结 果 DSA动脉瘤检出率为98.21%,MRA检出 率为94.64%,两者检出率相近,对比无统 计学意义(P>0.05);且MRA、DSA测定瘤体 直径及瘤颈宽度对比差异均无统计学意义 (P>0.05)。结论 采用MRA诊断颅内动脉瘤 准确性与DSA相近,且无创,能较好显示 动脉瘤三维构象与瘤体形态,可作为颅内 动脉瘤诊断的首选方案,但对MRA无法确 诊者,需辅以DSA检查。

Objective To compare the application value of magnetic resonance angiography (MRA) and digital subtraction angiography (DSA) in the diagnosis of intracranial aneurysms (IAN). Methods The clinical data of 52 patients with IAN confirmed by surgery and pathology in our hospital were retrospectively analyzed. All the patients underwent MRA and DSA after admission. The accuracy of MRA and DSA in the diagnosis of intracranial aneurysms was compared and the diagnostic value was analyzed. Results The detection rate of DSA in aneurysms was 98.21% and of MRA was 94.64% (P>0.05). The differences of MRA and DSA in measuring tumor diameters and neck width were not statistically significant (P>0.05). Conclusion The accuracy rate of MRA is close to that of DSA in diagnosis of intracranial aneurysms and they are noninvasive. They can well display the three-dimensional conformation and shape of aneurysms, which can be taken as the preferred diagnosis of intracranial aneurysms. For patients who can not be diagnosed by MRA, DSA should be adopted for help.

前言

颅内动脉瘤(IAN)多由脑动脉壁局部先天性缺陷或腔内压力上升所 引起,可发于任何年龄阶段,以中老年群体多见,发病率较高,仅次 于脑血栓及高血压脑出血,患者多预后差[1]。且IAN未破裂时患者通常 无明显症状,部分可伴头痛、癫痫、神经压迫等表现,而IAN破裂出血 后,患者可能有脑膜刺激征及颅内压增高征,血肿范围较大者,病情 恶化速度快,甚至可能出现脑疝[2],早期确诊IAN对患者治疗及预后改 善有积极作用。目前数字减影脑血管造影(DSA)被公认为诊断血管性疾 病的金标准,有其空间分辨率高的优势,可定位动脉瘤发生部位,测 定瘤体直径,显示血流动力学关系[3]。磁共振血管造影(MRA)则有其无 创、无辐射优势,可实现多方面观察,目前已广泛应用于临床。基于 此,为比较MRA、DSA在IAN诊断中的应用价值,我院对收治的52例患者 的临床资料进行了回顾性分析,现总结报道如下。
