3D-ASL脑灌注成像 在脑肿瘤诊断与分 级中的应用价值

摘要

目的 探讨3D动脉自旋标记成像 (ASL)全脑灌注成像技术在脑肿瘤诊断与 分级中的应用价值。方法 回顾性分析经 组织病理学及免疫组化证实的40例脑肿 瘤,包括胶质瘤14例(其中低级别胶质瘤 6例,高级别胶质瘤8例),脑膜瘤10例, 转移瘤10例,髓系肉瘤6例。测量3D-ASL 脑血流灌注图像肿瘤实质部分最大肿瘤 血流量值(TBFmax),对侧正常白质的脑血 流量值(CBF)及相对肿瘤血流量值rTBF。 rTBF=TBFmax/CBF。将脑肿瘤分为低级别 胶质瘤组、高级别胶质瘤组、脑膜瘤组、 转移瘤组及髓系肉瘤组,应用单因素方差 分析对各组间rTBF值进行比较,后续两两 比较采用SNK-q检验,P<0.05为差异有显著 性。结果 3D-ASL磁共振灌注测量得到的 脑肿瘤相对血流量值rTBF,髓系肉瘤组 rTBF低于低级别胶质瘤组、高级别胶质瘤 组、转移瘤组及脑膜瘤组,且差异均具有 统计学意义(P<0.05);低级别胶质瘤组 rTBF低于高级别胶质瘤组、转移瘤组及 脑膜瘤组,且差异均具有统计学意义(P <0.05);但高级别胶质瘤、脑膜瘤和转 移瘤三者之间rTBF差异无统计学意义(P> 0.05)。结论 3D-ASL能定量反映肿瘤微循 环的灌注情况,对脑肿瘤的诊断、鉴别诊 断及分级有重要价值。

Objective To study the three-dimensional arterial spin labeling (3D-ASL) whole brain perfusion imaging in the diagnosis of brain tumor and application value of classification. Methods Retrospective analysis confirmed by histopathological and immunhistochemical of 40 cases of brain tumors, including 14 cases of gliomas (6 cases with low grade gliomas and 8 high grade glioma), 6 cases of myeloid sarcoma, 10 cases of meningioma and metastatic tumor, respectively. Measure 3D-ASL cerebral blood flow perfusion image tumor substantial part of the largest value (TBFmax), the contralateral normal white matter of cerebral blood flow (CBF) value and the relative tumor blood flow(rTBF). rTBF=TBFmax/CBF. Glioma were divided into low and high grade, meningioma, metastatic tumor group and myeloid sarcoma group, using the single factor analysis of variance of rCBF value comparison between groups, using the snk-q test to compare two difference was statistically significant (P<0.05). Results The rCBF of 3D-ASL was obtained quantitative magnetic resonance perfusion imaging parameters, quantitative rTBF myeloid sarcoma group were lower than low grade glioma group, the high grade glioma group, metastatic tumors and meningiomas group, the differences were statistically significant, rTBF low grade glioma group were lower than the group of high grade glioma, meningioma and metastases group, the differences were statistically significant, high grade glioma, meningioma,and among metastases was no statistically significant difference. Conclusion 3D-ASL can quantitatively reflect the perfusion of the tumor microcirculation, diagnosis, differential diagnosis and classification of brain tumors has important value.

前言

脑肿瘤病理类型多种多样,各种级别的脑肿瘤治疗方案以及疾病 预后各不相同,恶性脑肿瘤预后较差,治疗困难且易复发,而良性脑 肿瘤以后良好,治疗容易,复发率低;故对脑肿瘤良恶性的鉴别及肿 瘤分级显得具有重要意义。肿瘤新生血管形成在肿瘤侵犯和转移中起 最关键作用,若没有血管生成,肿瘤瘤体直径仅能达到1~2mm。因 此,对瘤体血管生成进行定量评估具有重要意义。常规MRI增强扫描只 能显示血脑屏障的破坏程度。随着功能磁共振技术的发展,磁共振灌 注技术已成为反映组织微血管分布以及血流灌注的常规技术。   磁共振灌注成像技术包括两种。DSC-MRI已经广泛应用于脑肿瘤 的诊断及治疗后评价。3D-ASL是近年发展起来的全新容积灌注成像技 术,与传统的PET、DSC-MRI技术相比一些优势,已经有研究表明多次 磁共振增强检查后可使钆造影剂沉积于脑内[1],3D-ASL不需要注射对 比剂,可反复进行检查,因此其逐渐成为脑肿瘤的诊断、鉴别诊断、 治疗以及预后评估的新的方向。目前3D-ASL在脑肿瘤的研究主要集中 在胶质瘤的术前分级[2-4],其它肿瘤研究相对较少[5]。本研究的目的是 探讨3D动脉自旋标记成像(ASL)全脑灌注成像技术在脑肿瘤的诊断与分 级中的临床应用价值。
