CTA联合自适应统计 迭代重建技术在冠 状动脉病变患者中 的应用

摘要

目的 探讨CT血管成像(Computer tomographic angiography,CTA)联合 自适应统计迭代重建技术 ( A d a p t i v e statistical IR,ASIR)在冠状动脉病 变患者中的应用价值。方法 收集2012 年2月-2016年4月我院收治的78例冠状 动脉病变患者的临床资料,所有患者入 院均接受CTA或CTA联合ASIR成像检查, 其中38例接受常规CTA检查(A组),扫描 数据采用120 kVP联合滤波反投影重建 (Filtered back projection,FBP), 其余40例采用CTA+ASIR技术,应用ASIR 40%重建(B组),比较两组成像质量及患 者有效辐射剂量。结果 B组图像质量评 分为1分所占比例高于A组,其评分为2分 所占比例低于A组(P<0.05);B组信噪比 (Signal to noise ratio,SNR)、对比噪 声比(Contrast to noise ratio,CNR)均 高于A组,噪声低于A组(P<0.05);B组容 积CT剂量指数(CT dose index volume, CTDIvol)、剂量长度乘积(Dose length product,DLP)、有效剂量(Effective dose,ED)均低于A组(P<0.05)。结论 CTA 联合ASIR可提高冠脉病变患者成像质量, 降低患者受辐射剂量,安全可行。

Objective To investigate the application value of computer tomographic angiography (CTA) combined with adaptive statistical iterative reconstruction (ASIR) in patients with coronary artery disease. Methods The clinical data of 78 patients with coronary artery disease treated in our hospital from February 2012 to April 2016 were collected. All patients were examined with CTA or CTA combined with ASIR imaging. Among them, 38 patients were examined with CTA (A group), and the scanning data were reconstructed with 120 kVP combined with filtered back projection (FBP). The other 40 patients were examined with CTA combined with ASIR and the data were reconstructed with ASIR 40% (B group). The imaging quality and effective radiation dose of the two groups were compared. Results The proportion of image quality score of 1 point in B group was higher than that in A group, and the proportion of 2 points was lower than that in A group (P<0.05). The signal-to-noise ratio (SNR) and the contrast to noise ratio (CNR) in B group were higher than those in A group, and the noise was lower than that in A group (P<0.05). The CT dose index volume (CTDIvol), dose length product (DLP) and effective dose (ED) in B group were lower than those in A group (P<0.05). Conclusion CTA combined with ASIR can improve the imaging quality of patients with coronary artery disease and reduce the radiation dose of patients, which is safe and feasible.

前言

冠状动脉粥样硬化心脏病,即冠心病,系我国老年群体常见病, 且近年来发病趋向年轻化发展,无症状冠脉病变所占比例逐渐上升, 且早期检出率低[1]。目前用于冠脉病变筛查的方式主要包括侵入性 冠脉造影、冠脉内超声、运动试验、同位素心肌灌注扫描、静息心 电图等,选择性冠脉造影是目前诊断冠脉病变的金标准,但其为有 创操作,且相关并发症多,检查费用高昂,有其操作风险且无法显 示血管壁及斑块特点[2]。而冠脉CT血管成像(Computer tomographic angiography,CTA)则为冠脉病变无创检查的常用手段,其可较好显示 冠脉病变的严重程度,但有效辐射剂量高[3]。能谱CT成像技术的出现 使低浓度对比剂CTA检查成为可能,同时配合自适应统计迭代重建技术 (Adaptive statistical IR,ASIR)可在保证图像质量的同时降低辐射 损伤,提高临床筛查的安全性。基于此,为进一步证实CTA联合ASIR技 术在冠脉病变诊断中的应用价值,现对我院收治的78例患者的临床资 料进行了回顾性分析,报告如下。
