摘要
目的 探析磁共振弥散联合波谱 分析技术应用于脑肿瘤方面的诊断价值。 方法 选取2013年8月-2014年8月我院的40 例经手术和病理检查证实为脑肿瘤的患者 为研究对象,分别对患者给予常规磁共振 检查与磁共振弥散联合波谱分析检查,分 析对比两种检查方式的检查结果、表观 弥散系数以及波谱图,并分别计算出肿 瘤增强区和对侧正常区的NAA/Cho、NAA/ Cr和Cho/Cr比值等。结果 脑转移瘤的平 均ADC值为(12.08±2.92)×10-4mm2/s,脑 膜瘤的平均ADC值为(13.17±2.98)×10- 4 m m 2 /s,星形细胞瘤的平均ADC值为 (13.76±2.99)×10-4mm2/s。三种脑肿 瘤的ADC值均高于正常对侧脑组织,差 异具有统计学意义(均P>0.05),且三 种肿瘤任意选取两种比较,差异无统计 学意义(均P>0.05)。三种肿瘤的NAA均 较正常组织低,差异具有统计学意义 (均P<0.05),转移瘤与脑膜瘤的NAA/ Cho、NAA/Cr值明显低于星形细胞瘤(均P <0.05),三种肿瘤的Cr明显低于正常脑 组织(均P<0.05)。星形细胞瘤有4例Lac 峰,1例Lip峰;脑膜瘤有3例Lac峰,1例 Lip峰,1例Ala峰;转移瘤有2例Lac峰,5 例Lip峰。结论 磁共振弥散及波谱两种技 术均有其优势,若联合应用能更加敏感的 发现疾病以及精准的辨别疾病,值得临床 广泛推广使用。
Objective To explore the diagnostic value of magnetic resonance diffusion joint spectrum analysis technique in brain tumors. Methods From August 2013 to August 2014 in 40 cases confirmed by surgery and pathological examination for brain tumor patients as the research object, respectively, for patients with conventional magnetic resonance imaging, magnetic resonance (NMR) and diffuse check joint spectrum analysis, contrast analysis of two kinds of inspection mode of inspection results and apparent diffusion coefficient and the wave spectrum, and calculate the tumor enhancement region respectively and the contralateral normal area NAA/Cho, NAA/Cr and Cho/Cr ratio. And so on. Results The average ADC values for brain metastases (12.08+2.92)×10- 4 mm2 /s, the average ADC values for meningioma (13.17+2.98)×10-4mm2 /s, the average ADC values for astrocytoma was (13.76+2.99)×10-4mm2 /s. Three types of brain tumors of ADC values were higher than the contralateral normal brain tissue, the difference is statistically significant (P>0.05), and three kinds of tumor freely choose two kinds of comparison, there was no statistically significant difference (P>0.05). Three kinds of tumor of NAA was lower than those of normal group, the difference is statistically significant (all P<0.05), metastatic tumor with meningioma NAA/Cho, NAA/Cr value is significantly lower than that of astrocytoma (all P<0.05), three kinds of tumor Cr was lower than that of the normal brain tissue (both P<0.05). Astrocytoma with 4 cases of Lac peak, 1 case of Lip peak. Meningioma has 3 cases of Lac peak, 1 case of Lip peak, 1 case of Ala peak; In 2 cases of metastatic tumor Lac peak, 5 cases of Lip peak. Conclusion Magnetic resonance diffusion and spectrum analysis have their advantages and if the combined use of more sensitive and accurate to identify disease found disease, is worth to be widely used in clinical.

前言
磁共振弥散加权成像(DWI)已成为现今放射医学界较常用的一种 检查技术,氢质子磁共振波谱分析(1 HMRS)为近年来较新引进的技术, 敏感性较高,能够精准发现病灶,因此也逐渐成为较为可靠的检查技 术[1-2]。两者联合应用于检查中的新颖方式在国内较为少见,因此本文 选取2013年8月~2014年8月我院的40例经手术和病理检查证实为脑肿 瘤的患者为研究对象,在患者签署研究知情同意书、且经我院伦理会 批准的情况下,分析对比两种检查方式的检查结果、表观弥散系数以 及波谱图,并分别计算出肿瘤增强区和对侧正常区的NAA/Cho、NAA/Cr 和Cho/Cr比值,现分析报道如下。
