脑膜瘤患者应用CT灌注成像诊断及对脑膜瘤分型价值研究*

摘要

目的 通过观察与比较CT灌注成 像、MRI增强扫描用于脑膜瘤临床分型价 值,从而可为患者临床诊断及分型提供最 佳诊断方法。方法 将本院经病理诊断为 脑膜瘤的78例患者作为本次研究对象,并 分别经CT灌注成像与MRI增强扫描检查, 比较与分析两组不同方法所检测的结果, 同时对患者脑膜瘤分型及血流情况进行分 析。结果 经本次检测发现,与病理检查 比较,CT灌注成像临床诊断分型符合者为 71例,符合率为91.03%与MRI增强扫描检 查符合74例,占94.87%比较,无统计学意 义,P>0.05。经CT灌注成像检查发现,患 者脑血流量(CBF)、脑血容量(CBV)、平 均通过时间(MTT)、血管表明通透性(PS) 与其对侧脑组织比较,具有统计学意义, P<0.05;不同类型脑膜瘤患者CBF、CBV、 PS比较,具有统计学意义,P<0.05。结论 虽然临床应用MRI增强扫描与CT灌注成像 检查效果相当,但采用CT灌注成像可有效 获取患者脑膜瘤血供情况,从而为临床判 断脑膜瘤类型提供依据,并为临床评估患 者预后提供重要参考信息。

Objective Through observation and comparison of CT perfusion imaging, MRI scans used to enhance the value of the clinical classification of meningioma, which can be a clinical diagnosis of patients and provide the best diagnostic method for typing. Methods The hospital pathologically diagnosed as meningioma 78 patients in this study and were treated with CT perfusion imaging and enhanced MRI scan results, comparison and analysis of two different methods to detect, while typing meningioma patients and blood were analyzed. Results After this test found that compared with the pathological examination, CT perfusion imaging in the clinical diagnosis of type consistent with those for the 71 cases, the rate was 91.03% and enhanced MRI scan consistent with 74 cases, accounting for 94.87% compared with no significant difference, P>0.05. By CT perfusion imaging examination revealed that the patient of cerebral blood flow (CBF), cerebral blood volume (CBV), mean transit time (MTT), vascular permeability show (PS) and its contralateral brain tissue compared with statistical significance, P<0.05; meningioma patients with different types of CBF, CBV, PS compared with statistical significance, P<0.05. Conclusion Although the clinical application of enhanced MRI and CT perfusion imaging results are equivalent, but the use of CT perfusion imaging in patients with meningiomas can effectively get the blood supply, and thus provide the basis for clinical judgment meningioma type and prognosis for clinical assessment provide important reference information.

前言

临床上,脑膜瘤是人体颅内一种常见性肿瘤疾病,这种疾病临床 发病率大约占颅内肿瘤的19.0%,且发病年龄大多>45岁,极易发生于 女性人群中。由于这种疾病发病较为缓慢,且病程长,因此临床初步 诊断时一般需根据患者临床表现及全身等检查,同时还需结合CT及MRI 等了解患者肿瘤位置及大小等。术前通过影像学检查可为临床提供肿 瘤部位和肿瘤大小及分型等信息,从而为临床手术治疗方案选择提供 参考依据,最终可保障手术成功。然目前对CT灌注成像与MRI增强扫描 检查临床应用价值尚存争议,因此本次为探讨2者对脑膜瘤临床分型诊 断价值,特选择本院所收治的脑膜瘤患者进行研究,如下报道。
