胸部HRCT联合18F-FDG PET-CT双时相显像对肺内孤立性磨玻璃结节的诊断价值*

摘要

目的 探讨胸部HRCT联合18F-FDG PET-CT双时相显像对肺内孤立性磨玻璃结节的诊断价值。方法 回顾性分析2015年6 月至2017年1月间30例经病理证实的肺内孤立性磨玻璃结节患者,先后行胸部HRCT和18F-FDG PET-CT双时相显像,所 得结果分别与病理检查结果对照比较。结果 恶性肺内孤立性磨玻璃结节常规与延迟显像SUVmax显著高于良性病变 患者,P<0.05,差异有统计学意义;胸部HRCT和18F-FDG PET-CT检查两者联合的灵敏度、特异度、阳性预测值、阴 性预测值、准确率明显高于其他两者,P<0.05,差异有统计学意义。结论 胸部HRCT和18F-FDG PET-CT检查两者联合 能够优势互补,可以显著提高肺孤立性单纯性磨玻璃结节的诊断准确率,降低误诊率。

Objective To explore the diagnostic value of the chest HRCT combined with 18F-FDG PET-CT in the diagnosis of isolated pulmonary abrasion of the lung. Methods Retrospective analysis of 30 pathologically confirmed cases of pulmonary isolated glass nodules from June 2015 to January 2017, the dual-phase imaging findings of HRCT and 18FFDG PET-CT were performed successively, the results were compared with the results of pathological examination. Results The conventional and delayed imaging SUVmax in malignant lung is significantly higher than that in patients with benign lesions, P<0.05, the difference was statistically significant, the sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the combined chest HRCT and 18F-FDG PET-CT were significantly higher than those of the other two, P<0.05, the difference was statistically significant. Conclusion Chest HRCT and 18F-FDG PET-CT examination can complement with each other, it can significantly improve the diagnostic accuracy of pulmonary isolated single-pure glass nodules, lower misdiagnosis rate

前言

孤立性肺结节(solitary pulmonary nodule, SPN)是指肺内单发圆形或类圆形病灶,其直径较 小,边界清楚,周围为含气肺组织所包绕的肺部结 节病变,其影像学特征复杂,是胸部影像学诊断难 点[1-4]。肺内孤立性肺结节的恶性率较高,因此胸部 HRCT联合18F-FDG PET-CT双时相显像对肺内孤立性磨 玻璃结节的诊断价值对患者的预后具有重要的临床意 义[5],本研究对2015年6月至2017年1月间30例经病理 证实的肺内孤立性磨玻璃结节患者的影像资料进行分 析,现报告如下。
