摘要
目的 探讨高分辨率CT扫描在提高诊断支气管扩张敏感性、准确性的应用价值。方法 选取2010年1月至2014年6月期 间我院临床和手术病理证实的支气管扩张患者60例,依据随机分配原则分为CT组和高CT组,依据肺段支扩表现分为 柱状型、囊状型、混合型和曲张型,CT组给予常规CT扫描检测,高CT组给予高分辨率CT扫描检测,统计分析所有患 者的CT表现,采用ROC曲线分析二种CT扫描对支气管扩张的鉴别诊断价值。结果 高CT组支气管扩张检出率明显高 于CT组,差异有统计学意义(P<0.05);高CT组鉴别柱状型、囊状型、混合型和曲张型支气管扩张准确率明显高 于CT组,差异有统计学意义(P<0.05);ROC曲线结果显示,CT扫描鉴别诊断支气管扩张的敏感性为81.26%,特异 性为78.69%,准确性为84.56%,高分辨率CT扫描诊断支气管扩张的敏感性为89.12%,特异性为86.24%,准确性为 95.28%。结论 高分辨率CT扫描可有效提高支气管扩张的检出率和鉴别疾病类型的准确率,在鉴别诊断支气管扩张 时具有更高的敏感性、准确性,值得临床作进一步推广。
Objective To discuss the application value of high resolution CT scan in increase the diagnosis of bronchiectasis sensitivity, accuracy. Methods 60 patients with bronchiectasis were selected from January 2010 to June 2014 in our Hospitals, according to the random distribution, all patients were divided into CT group and high CT group, based on the expansion of CT pulmonary segments performance,all patients were divided into cylindrical type, cystic type, mixed type and varicose, CT group was given conventional CT scanning, high CT group was given high resolution CT scan, analysed all patients’ CT manifestation,and used ROC curve analysis to analysis two kinds of CT scanning of value in the differential diagnosis of bronchiectasis. Results High CT group Bronchiectasis detection rate was obviously higher than CT group, the difference was statistically significant (P<0.05); High CT group to identify high columnar type, cystic type, mixed type and varicose bronchiectasis accuracy rate was significantly higher than CT group(P < 0.05); ROC curve results showed that, CT scan differential diagnosis of bronchiectasis sensitivity of 81.26%, specificity of 78.69%, the accuracy of 84.56%, high resolution CT scan differential diagnosis of bronchiectasis sensitivity of 89.12%, specificity of 86.24%, the accuracy of 95.28%. Conclusions High resolution CT scan can effectively improve the accuracy in the detection rate and the identification of bronchiectasis disease type, in the differential diagnosis of bronchiectasis with higher sensitivity and accuracy,it’s worth for further clinical promotion.

前言
支气管扩张是呼吸系统最常见的慢性化脓性疾病 之一,是指支气管及其周围肺组织慢性化脓性炎症和 纤维化,造成支气管壁的肌肉和弹性组织受到损伤, 引起支气管变形和持续性扩张的病变[1]。以往,支气 管扩张的诊断主要依据临床表现、X线胸部平片和支气管造影,尤其支气管碘油造影被认为是诊断该疾病 的金标准,但创伤较大且操作复杂[2]。近年来,随着 CT技术的发展,CT扫描逐渐取代以往的诊断手段,可 有效显示患者支气管形态,但由于容积效应和呼吸运 动等因素,在总体诊断的敏感性和准确性方面较为不理想[3-4]。而高分辨率CT扫描在扫描时,可更为清 晰显示支气管的特殊形态和表现,有助于提高对支气 管扩张诊断的敏感性和准确性[5]。对此,我院通过对 支气管扩张患者给予高分辨率CT扫描和常规CT扫描检 测,以探讨高分辨率CT扫描在鉴别诊断支气管扩张中 的作用和价值,现将结果报道如下。
