高分辨率磁共振诊断缺血性脑血管病的临床研究

摘要

目的探讨高分辨率磁共振诊断 缺血性脑血管病的临床应用价值。方法 选取2016年2月-2017年2月我院收治的180 例疑似缺血性脑血管病患者为研究对象, 均行高分辨率磁共振扫描检查,观察缺 血性脑血管病影像学表现,并以数字减影 血管造影(DSA)检查结果为金标准,评估 高分辨率磁共振诊断缺血性脑血管病的效 能。结果 缺血性脑血管病不同分类经高 分辨率磁共振扫描检查均具有典型影像 学表现,高分辨率磁共振诊断缺血性脑 血管病的灵敏度、特异度、准确度各为 90.16%、79.31%、86.67%;偏心性狭窄的 大脑中动脉管壁,经高分辨率磁共振T2WI 检查提示靠近管腔的高信号带为纤维帽, 缺血性脑血管病经高分辨率磁共振扫描提 示左侧大脑中动脉长条形斑块伴强化,缺 血性卒中高分辨率磁共振成像矢状位平扫 和增强序列图像提示左侧大脑中动脉不稳 定斑块伴重度狭窄。结论 高分辨率磁共 振诊断缺血性脑血管病的临床应用价值较 高,其可作为缺血性脑血管病的一种安全 有效诊断手段。

Objective To investigate the clinical value of high resolution magnetic resonance imaging (MRI) in the diagnosis of ischemic cerebrovascular diseases. Methods 180 patients with suspected ischemic cerebrovascular diseases who were admitted to the hospital from February 2016 to February 2017 were selected as the subjects. All patients were examined by high-resolution MRI. The imaging findings of ischemic cerebrovascular diseases were observed, and the efficiency of high-resolution MRI in the diagnosis of ischemic cerebrovascular diseases was evaluated with the results of digital subtraction angiography (DSA) as the golden standard. Results The high-resolution MRI findings of different types of ischemic cerebrovascular diseases were typical, and the sensitivity, specificity and accuracy of high resolution MRI in diagnosing ischemic cerebrovascular diseases were 90.16%, 79.31% and 86.67%, respectively.The high-resolution MRI T2WI examination of cerebral middle artery with eccentric stenosis showed that high-signal zone close to the lumen was the fibrous cap, and high-resolution MRI of ischemic cerebrovascular diseases showed that there was enhancement of strip plaques in left middle cerebral artery. The high-resolution MRI sagittal plain scan and enhanced sequence images of ischemic stroke showed that the left middle cerebral artery unstable plaques with severe stenosis. Conclusion High resolution MRI is of high value in the diagnosis of ischemic cerebrovascular diseases. It can be used as a safe and effective diagnostic method for ischemic cerebrovascular diseases.

前言

缺血性脑血管病又被称为脑缺血性疾病,属于不同程度缺血性脑 血管疾病的总称,已成为全球范围内危害人类健康的常见疾病,其有 较高病死率和致残率,近年来其患病率有逐年增高趋势且日趋年轻 化,现代医学表明颅内外血管粥样硬化、血管多处狭窄闭塞是引发缺 血性脑血管疾病的常见因素,临床尽早为患者实施合理干预措施对 降低本病致残率和病死率尤为重要[1-2]。一直以来数字减影血管造影 (DSA)被认为是鉴别诊断缺血性脑血管疾病的有效手段,并且一度成 为其诊断的金标准,但该诊断方案费用较高,且属于有创检查,较难 于临床上广泛推广应用[3];颈动脉超声、颅多普勒超声因具有重复性 好、费用低、创伤小等优点,在脑血管疾病诊断中的应用日益广泛, 但临床实践证实这些方案单一应用时的诊断准确率较低[4]。近来随着 磁共振扫描技术不断提高和新型线圈普及应用,高分辨率磁共振成像 逐渐出现并应用于颅内外血管疾病检查中,但目前有关高分辨率磁共 振诊断缺血性脑血管病的临床应用价值尚缺乏统一定论,因而笔者于 本文展开临床回顾性分析,分析结果如下。
