MSCT在嗜铬细胞瘤 诊断、鉴别及转移 评估中的应用

摘要

目的 探讨多层螺旋CT(MSCT)在 嗜铬细胞瘤诊断、鉴别及转移评估中的 应用。方法 回顾性分析在我院住院使用 131I-MIBG(131I-间碘苄胍)治疗的74例嗜铬 细胞瘤患者的MSCT影像学资料,总结CT 影像学表现及良恶性肿瘤鉴别诊断。结 果 MSCT诊断准确率为95.94%,敏感性为 93.75%,特异性为95.55%。良恶性嗜铬细 胞瘤大小及平扫期、动脉期、静脉期CT值 差异显著,具有统计学意义(P<0.05)。良 恶性嗜铬细胞瘤均伴随囊变坏死,增强扫 描增强恶性病变均存在包膜浸润,随访发 现均出现不同位置的转移,部分伴随多发 转移。结论 MSCT可作为嗜铬细胞瘤早期 诊断、良恶性鉴别诊断及肿瘤远处转移的 重要影像学方式。

Objective To investigate the application of multi-slice spiral CT (MSCT) in the diagnosis, differentiation and metastasis evaluation of pheochromocytomas. Methods The MSCT imaging data of 74 hospitalized patients with renal pheochromocytomas treated by 131I-MIBG (131I-metaiodobenzylguanidine) in Nuclide Diagnosis and Treatment Center in our hospital were retrospectively analyzed. The CT findings and differential diagnosis of benign and malignant tumors were also analyzed. Results The diagnostic accuracy of MSCT was 95.94%, the sensitivity was 93.75% and the specificity was 95.55%. The differences in size, plain scan phase, arterial phase, venous phase CT values between benign and malignant pheochromocytoma were significant (P<0.05). Benign and malignant pheochromocytomas were complicated with cystic degeneration or necrosis. The enhanced scan was enhanced. Malignant ones were with capsular infiltration. Follow-up found that there was metastasis at different sites and some were complicated with multiple metastasis. Conclusion MSCT can be used as an important imaging method for diagnosis and differential diagnosis of benign and malignant pheochromocytomas and distant metastasis.

前言

嗜铬细胞瘤在临床并不少见,典型症状为阵发性高血压、血儿茶 酚胺升高等,但部分为非功能性嗜铬细胞瘤,临床诊断较为困难。大 部分患者嗜铬细胞瘤为良性,手术切除即可;5%~26%为恶性嗜铬细胞 瘤,临床表现和影像学检查缺乏特异性,尤其在病灶周围血管未受侵 犯情况下,诊断难度较大[1]。随着CT影像学发现,MSCT逐渐用于嗜铬 细胞瘤的诊断,具有扫描时间短、容积采集、空间分辨率高等优点, 有利于提高诊断准确率。对此,本文回顾性分析了74例嗜铬细胞瘤 MSCT诊断情况及影像学表现,探讨其在疾病鉴别诊断中的应用价值, 现报道如下。
