磁共振LAVA动态增强半定量分析对前列腺癌的诊断价值*

摘要

目的 通过磁共振LAVA动态增强 (DCE-MRI)相关参数的分析,探讨DCE-MRI 对前列腺癌的诊断价值。方法 对经组织 病理学证实的29例PCa和58例BPH患者行肝 脏快速容积采集序列(LAVA)多期动态增强 扫描,获得SI-T曲线,并计算出峰值时 间(Tmax)、最大信号强度(Slmax%)、强化 率(R)、流出分数(washout),比较各参 数在PCa、gBPH、sBPH之间的差异,并进 行ROC曲线分析。结果 PCa组SI-T曲线以 速升下降型为主,gBPH、sBPH组以平台型 为主;Tmax、R、Washout三组组间差异有 统计学意义(P<0.05),Slmax%组间差异无 统计学意义(P>0.05);Tmax、R、Washout 值gBPH与sBPH分组间差异无统计学意义 (P>0.05);Tmax的ROC曲线下面积最大,诊 断敏感度及特异度分别为93.6%、84.6%; PCa组Tmax多在1分钟内(75.9%),sBPH组Tmax 多在1-2分钟内(55.5%),而gBPH组Tmax多 在两组区间,即1-2分钟(42.5%)及2分钟 后(40.0%)。结论 DCE-MRI半定量分析在 前列腺癌诊断中具有较高价值,可用于前 列腺良恶性病变的鉴别诊断。

Objective By analyzing the LAVA dynamic enhanced magnetic resonance (DCE-MRI) related parameters to evaluate the diagnostic value of DCE-MRI for prostate cancer. Methods 29 patients with PCa and 58 BPH patients with liver histopathology confirmed the line fast volume acquisition sequence (LAVA) multiphase dynamic enhanced scan, access SI-T curve, and calculate the peak time (Tmax), the maximum signal strength (Slmax %), enhancement ratio (R), out of scores (washout), compare the differences in the parameters between PCa, gBPH, sBPH of, and ROC curve analysis. Results PCa group SI-T curve at a speed drop liter main type, gBPH, sBPH platform-type-based group; between Tmax, R, Washout three groups was statistically significant (P<0.05) differences between the groups was not Slmax% statistically significant (P>0.05); Tmax, R, Washout value between gBPH and sBPH group showed no significant difference (P>0.05); area under the ROC curve Tmax largest diagnostic sensitivity and specificity were 93.6%, 84.6%; PCa group within Tmax more (75.9%) one minute, sBPH group more in Tmax (55.5%) for 1-2 minutes, and two more in gBPH group Tmax range, ie 1-2 minutes (42.5%) and after 2 minutes (40.0%). Conclusion Semi-quantitative analysis of DCE-MRI has a higher value in the diagnosis of prostate cancer can be used for differential diagnosis of benign and malignant prostate lesions

前言

良性前列腺增生(Benign prostatic hyperplasia,BPH)和前列腺 癌(Prostate cancer,PCa)是老年男性泌尿系统中较为常见的疾病。 各种检查对于二者的鉴别优缺点不同,磁共振由于较好的软组织分辨 率,在二者的鉴别诊断中得到了广泛的的运用。不过由于不同类型的 增生组织及Pca病理组织成分不同,在磁共振上的信号有一定的重叠。 本研究通过运用肝脏容积加速成像(Liver Acquisition with Volume Acceleration,LAVA)序列,探讨MR动态增强扫描(Dynamic Contrast Enhanced MR imaging,DCE-MRI)的半定量参数分析对前列腺癌的诊断 价值。
