磁共振诊断乳腺疾病的临床价值研究

摘要

目的评价磁共振对乳腺疾病的 诊断价值,探讨磁共振检查乳腺病变的临 床适应性,为进一步临床应用提供可参 考意见。方法 收集我院乳腺疾病患者95 例,进行多时项增强磁共振检查,对病灶 形态边缘等形态学特征进行描述,测定血 流动力学参数获得病变早期强化率,分析 时间-信号强度曲线,综合形态、早期强 化率和时间-信号强度曲线分型对病变进 行评价。结果 纳入病例中恶性病变为43 例,发现43个病灶,占41.75%,良性病变 52例,发现60个病灶,占58.25%。恶性病 变表现形态不规则、与周边组织分界不清 为35例,占81.40%;良性病变表现为圆形 或椭圆形、边界光整41例,占68.33%。恶 性病变组不规则、分界不清显著高于良性 病变,P<0.05。恶性病变组III型显著高 于良性病变组,且I型显著低于良性病变 组,P<0.05。乳腺恶性病变平均早期强化 率为167.52±69.34%,良性病变平均早期 强化率为97.40±65.85%。恶性病变中早 期强化率超过100%病例数显著高于良性病 变,P<0.05;小于50%和50-100%病例数显 著低于良性病变,P<0.05。结论 应用动 态增强磁共振诊断乳腺病变仍需综合病变 形态学、时间-信号强度曲线以及早期强 化率,综合作出判断。

Objective To evaluate the diagnostic value of magnetic resonance imaging in the diagnosis of breast diseases, and to investigate the clinical applicability of magnetic resonance imaging in the diagnosis of breast lesions, and to provide reference for further clinical application. Methods 95 patients with breast disease from January 2012 to 2015 March in Oncology department and Department of thoracic surgery of our hospital were collected and performed preoperative magnetic resonance imaging (MRI). The patients were suffered multi-item enhanced magnetic resonance imaging (MRI), to describe the morphological characteristics of lesion morphology edge, the determination of hemodynamic parameters obtained early lesions of the rate and the time signal intensity curve analysis, morphology, early and intensive rate and time signal intensity curve types of lesions. Results Malignant lesions were found in 43 cases, 43 lesions were found, accounting for 41.75%, 52 cases of benign lesions, 60 lesions were found, accounting for 58.25%. The malignant lesions showed irregular shape, and the peripheral tissue boundary was not clear in 35 cases, accounting for 81.40%; benign lesions showed round or oval, boundary light in 41 cases, accounting for 68.33%. The malignant lesions were not significantly higher than those in benign lesions, P<0.05. III in malignant lesions group was significantly higher than that in benign lesion group, and I was significantly lower than that in benign lesion group, P<0.05. The early enhancement rate of breast malignant lesions was 167.52 69.34%, the average early enhancement rate of benign lesions was 97.40 65.85%. The early enhancement rate of malignant lesions was significantly higher than that of benign lesions, P<0.05; less than 50% and 50-100% were significantly lower than those of benign lesions, P<0.05. Conclusion Application of dynamic enhanced magnetic resonance imaging in the diagnosis of breast lesions is still required to synthesize the lesion morphology, time signal intensity curve and early enhancement rate.

前言

乳腺疾病是妇女常见病,其中乳腺癌是女性多发病,且近年发病 率显著升高,严重患者生命健康。调查研究表明乳腺癌不仅发病率逐 年升高,而且发病年龄呈现年轻化[1]。影像学检查对于乳腺疾病的检 出意义重大,触诊较难发现微小病灶,而影像学则较易发现微小病 灶,利于早期诊断、早期治疗,减少中晚期才发现病灶。影像学检查 方法包括X射线、B超以及磁共振等。X射线是常见筛查乳腺疾病的手 段,具有快速、方便、费用低等特点,可较好的观察乳腺病变钙化情 况,但是受乳腺腺体的影响。致密或囊性改变的腺体可遮掩病变,出 现假阳性,降低检出率[2]。B超具有无辐射性和无检查盲区优势,可 检测乳腺内部结构,但检测的准确性取决于设备和检查医师的个人经 验,因此在诊断良心恶性乳腺病变具有一定的局限性[3]。磁共振不受 乳腺组织密度的影响,对软组织具有较好的分辨率,无创性、无辐射 性,可以多参数、多系列、多方位诊断乳腺疾病。动态磁共振增强可 以更为清晰的显示病变形态学、周围组织关系,注射对比剂获得血流 动力学,绘制动态增强曲线,提高检出敏感性。本研究收集我院2012 年1月至2015年3月肿瘤科和胸外科就诊,且术前行磁共振检查的乳腺 疾病患者95例,进行磁共振多时相检查,探讨其对乳腺良恶性诊断鉴 临床适应性和临床价值,为进一步临床应用提供可参考意见。
