PC-MRI对三脑室底造瘘术后脑脊液循环的研究

目的通过相位对比磁共振电影 成像技术观察第三脑室底造瘘术瘘口区脑 脊液流动动力学情况,并与正常人中脑导 水管脑脊液流动情况作对比,从而评估手 术疗效。方法 使用Siemens Avanto 1.5T MR对30例正常人及12例行三脑室底造瘘术 患者分别行导水管及造瘘口PC-MRI扫描, 利用Argus软件进行数据测量及分析,测 量其在一个心动周期内脑脊液流动的各参 数,如峰值流速、平均流速、净流量、平 均流速率等。结果 30例正常人中脑导水 管脑脊液流动各数值(收缩期峰值流速、 舒张期峰值流速、净流量)与12例造瘘术 后瘘管区脑脊液流动相关参数进行独立t 检验,P值均大于0.05。结论 第三脑室底 造瘘口脑脊液流动形式与正常人相似,呈 周期搏动样流动。心脏收缩期脑脊液从瘘 管口上端流向下端,舒张期脑脊液从瘘管 口下端流向上端。造瘘术可重建脑脊液循 环通道,使之达到正常中脑导水管的生理 特点。
Objective To study and analyze cerebrospinal fluid (CSF) flow in patients with endoscopic third ventriculostomy (ETV) compared with volunteers, to assess the surgery efficacy by investigating. Methods 30 volunteers and 12 patients with endoscopic third ventriculostomy were selected. CSF flow was measure inside the cerebral aqueduct and the stoma area separately with cine PC (Siemens Avanto 1.5T) in all case. All diffusiontensor data (Systolic Velocity、Average Velocity、Net Flows、Average Flow Rate) were processed with Argus software on workstation. Results Independent t test was processed on PSV、DSV and NF between normal and ventriculostomy and found that P >0.05 in all three tests. Conclusions The CSF flow pattern in patients performed with ventriculostomy in the floor of the third ventricle is similar with the normal, presenting as bidirectional periodical pulsating waves in the fistula. The cerebrospinal fluid circulation can be rebuilt by ETV, so that its normal physiological requirements can be satisfied. CSF flow waveforms analysis by MR cine PC seems to be a reliable, reproducible, and sensitive method for assessing the CSF dynamics.

前言
脑积水为临床常见病,内镜下第三脑室底造瘘术(endoscopic third ventriculostomy,ETV)创伤小,现已被广泛应用于梗阻性 脑积水的治疗[1]。相位对比磁共振电影(phase-contrast magnetic resonance imaging,PC-MRI)能准确地分析脑脊液流动的速度及方 向,为脑脊液循环(cerebral spinal fluid circulation,CSF)的研 究提供了有效的依据,亦为第三脑室底造瘘口脑脊液循环的研究提供 了技术支持,从而评判第三脑室底造瘘术的手术疗效。
