摘要
目的 探究C反应蛋白(CRP)、癌胚抗原(CEA)、α-L-岩藻糖苷酶(AFU)联合检测在肝癌早期诊断中的价值。方法 回 顾性分析2014年3月-2017年3月期间我院收治的62例早期原发性肝癌患者(肝癌组)、74例肝硬化患者(肝硬化组)及 60例健康体检者(对照组)的临床资料。比较三组受试者血清CRP、CEA、AFU水平差别,分析三项联合检测、双项联 合检测、单项检测诊断准确性差异。结果 肝癌组与肝硬化组CRP、CEA、AFU水平均明显高于对照组,且肝癌组明显 高于肝硬化组(P均<0.05)。CRP、CEA、AFU三项指标联合检测诊断准确率为93.6%(58/62),明显高于各单项检测的 诊断准确率(P均<0.05)。结论 CRP、CEA及AFU联合检测能有效提高肝癌早期诊断准确性,对患者早就诊、早治疗 有积极意义。
Objective To investigate the values of combined detection of C reactive protein (CRP), carcinoembryonic antigen (CEA) and α-L-fucosidase (AFU) in the early diagnosis of hepatocellular carcinoma (HCC). Methods From Mar. 2014 to Mar. 2017, the clinical data of 62 patients with early primary hepatocellular carcinoma (HCC group), 74 patients with liver cirrhosis (cirrhosis group) and 60 healthy subjects (control group) were retrospectively analyzed. The differences of serum CRP, CEA and AFU between the three groups were compared, and the difference of the diagnostic accuracy between the joint detection of three indicators, joint detection of two indicators and detection of single indicator was analyzed. Results The levels of CRP, CEA and AFU in the HCC group and cirrhosis group were significantly higher than those in the control group, and the levels of CRP, CEA and AFU in the HCC group was significantly higher than that in the cirrhosis group (P<0.05). The diagnostic accuracy of joint detection of CRP, CEA and AFU was 93.6% (58/62) which was significantly higher than that of single detection (P<0.05). Conclusion The combined detection of CRP, CEA and AFU which has positive significance for early diagnosis and early treatment, can effectively improve the accuracy of early diagnosis of hepatocellular carcinoma.

前言
原发性肝癌是临床最常见的恶性肿瘤类型,死亡 率仅次于肺癌与胃癌,通常按照癌变部位不同主要分 为肝细胞癌(hepatocellular carcinoma,HCC)与肝 内胆管细胞癌(intrahepatic cholangiocarcinoma, IHCC)。由于多数患者就诊时已发展为中晚期癌症, 已经丧失最佳的治疗时机,目前肝癌患者的5年生存 率仅在5%左右[1],因此早期诊断是延长肝癌患者生存 期的关键所在。临床常用的影像学诊断方法多费用高 昂,且影响检查结果的不可控性因素过多;而穿刺活 检为有创操作,且采样难度较高,不易为患者所接 受。对此,本研究旨在探讨C反应蛋白(C reactivep r o t e i n , C R P )、癌胚抗原 ( c a r c i n o e m b r y o n i c antigen,CEA)、α-L-岩藻糖苷酶(α-L-fucosidase, AFU)联合检测对肝癌早期诊断的价值,现报道如下。
