2型糖尿病患者骨质疏松的相关因素分析研究*

摘要

目的 在2型糖尿病(T2DM)合并骨质疏松症的患者中针对发病的相关因素进行回顾性分析。方法 骨质疏松组:T2DM 合并骨质疏松症患者59例,平均年龄28-80岁;非骨质疏松组:骨密度正常的2 型糖尿病患者61例,平均年龄31-79 岁。入组患者均测定糖化血红蛋白、股骨骨密度、腰椎骨密度、骨质疏松四项,然后对两组患者的性别、年龄、糖 尿病病程、糖化血红蛋白、体重指数(BMI)、腰椎骨密度、股骨骨密度、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)、 Ⅰ型前胶原N-端前肽(PINP),25羟基维生素D(VitD),骨钙素(BGP)的差异性进行分析,及对引起股骨骨密度下降的危 险因素进行探讨。结果 T2DM合并骨质疏松患者中股骨骨密度下降明显,伴有PINP与BGP的升高(P=0.015、0.011、 0.042)。骨质疏松组中股骨骨密度下降与患者年龄、糖尿病病程负相关(P=0.002、0.009);与糖化血红蛋白,βCTX、PINP、VitD、BGP无显著相关(P值>0.05)。结论 T2DM患者中性别,年龄,糖尿病病程与骨质疏松症相关。

Objective To investigate the influence of related facters on T2DM patients combined with osteoporosis. Methods 59 patients with T2DM aged 28-80 years old asscoicated with osteoporosis were selected, and 61T2DM patients aged 31-79 years old with normal bone mineral density as the control. The bone mineral density of all patients including lumbar spine, femoral neck were measured. The correlation analysis of gender, age, duration of diabetes, glycosylated hemoglobin, body weight index, β-CTX, PINP, VitD, BGP and BMD was performed. Results The bone mineral density of femoral neck in osteoporosis group was significantly lower than that of the control group, while PINP and BGP were significantly higher(P<0.05). In osteoporosis with T2DM groups, age and duration of diabetes showed negative correlation with femoral neck BMD(P<0.01),while glycosylated hemoglobin, body weight index, β-CTX, PINP=VitD =BGP were found no significant correlation. Conclusion Femoral neck BMD showed a significantly decline inT2DM patients with PINP and BGP elevation. Meanwhile female, older or long course in patients T2DM increased the incidence of osteoporosis. To enhance detecting BMD in T2DM is helpful to prevent the occurrence of osteoporosis to a certain extent.

骨质疏松的潜在发病已成为影响糖尿病患者生命 安全的隐形杀手。王合明2010年报道深圳市人群体检 测得骨质疏松比例为37%[1],该数据进一步提示骨质 疏松严重威胁深圳地区中老年人尤其是2型糖尿病患 者的健康。为此,本研究针对深圳市的T2DM合并骨质 疏松患者可能的危险因素进行相关分析。
