摘要
目的 研究多模态功能磁共振 (MRI)成像对脑瘫儿童诊治的指导作用。 方法 收集2015年1月-2017年1月年间于我 院确诊为脑瘫的62例患儿为研究对象,分 析其颅脑多模态功能磁共振成像特点, 评估多模态功能磁共振检查在脑瘫患儿 诊断中的应用价值。结果 与常规MRI扫 描比较,磁敏感加权成像技术(SWI)脑 实质出血及蛛网膜下腔出血检出率更高 (P<0.05);DTI对脑内异常信号检出率显 著高于常规MRI(P<0.05)。结论 多模态功 能磁共振成像可有效弥补常规MRI序列的 不足,有效提高脑出血及其他脑内信号异 常检出率,在脑瘫的诊断中具有良好的应 用价值,值得临床推广
Objective To study the guiding role of multimodal functional magnetic resonance imaging (MRI) in the diagnosis and treatment of children with cerebral palsy. Methods A total of 62 cases of children diagnosed as cerebral palsy in our hospital from January 2015 to January 2017 were analyzed retrospectively and were included into the study. The features of multimodal functional magnetic resonance imaging were analyzed, and the application value of multimodal functional magnetic resonance imaging was evaluated in the diagnosis of children patients with cerebral palsy. Results Compared with conventional MRI scan, the detection rates of intra-parenchymal hemorrhage and subarachnoid hemorrhage were higher in susceptibility weighted imaging(SWI) (P<0.05). The detection rate of abnormal brain lesions of DTI was significantly higher than that of conventional MRI (P<0.05). Conclusion Multimodal functional magnetic resonance imaging can effectively compensate for the deficiencies of conventional MRI sequence, and improve the detection rates of cerebral hemorrhage and abnormal brainsignals effectively, and it has good application value in screening children patients with cerebral palsy, and is worthy of clinical promotion.

前言
小儿脑瘫属于儿童严重残障,为患儿家庭及社会带来沉重的负 担,中枢性运动功能障碍、肌张力降低、病理性反射等特异性临床 表现是疾病诊断的主要依据[1]。患儿自主活动力低,临床表现特异度 低,疾病确诊难度大,影像学检查在疾病诊断中起着至关重要的作 用,MRI检查无创伤、无辐射,可进行多方位、多序列成像,清晰显 示脑干形态、结构与周围结构情况,对大脑皮质、白质有良好的分辨 率,是小儿脑瘫的常见辅助性诊断工具[2]。随着扩散张量成像、灌注 成像等特殊磁共振成像技术的不断发展,多模态功能磁共振成像逐渐 应用于脑瘫患儿的诊断[3],为研究多模态功能磁共振成像在脑瘫儿童 诊治中的应用效果,我院开展如下研究。
