骨盆部IDEAL-IQ及DWI成像对女性贫血的诊断价值*

目的探讨骨盆诸骨ADC值及FF 值与女性贫血程度的关系,评价磁共振 IDEAL-IQ及DWI序列诊断女性贫血的可行 性。方法 回顾性分析40例女性贫血患者 和40例女性正常志愿者的影像资料,分 别测量骨盆诸骨(耻骨、髂骨、骶骨和坐 骨)的ADC值及FF值,比较组间各参数值的 差异并采用受试者工作特征(ROC)曲线评 价各参数值的阈值及诊断效能。结果 同 年龄段正常组的ADC值和FF值均高于贫血 组,中年组ROC曲线下面积分别是0.978、 0.764,诊断阈值分别为0.646×10-3mm2 / s、67.10%,老年组ROC曲线下面积分 别是0.819、0.897,诊断阈值分别为 0.578×10-3mm2 /s、59.80%;轻度贫血组 的ADC值和FF值均高于中、重度贫血组(P <0.05),中、重度贫血组间各参数值没 有统计学差异;骨盆不同解剖部位间各参 数值无统计学差异。结论 磁共振IDEALIQ及DWI成像能够从骨髓成分变化的角度 间接反映女性是否贫血,并可初步判定贫 血程度。
Objective To investigate the diagnostic value of ADC value and FF value of female pelvic bone with iron deficiency anemia. Methods Analyzed the MR images of 40 patients with anemia and 40 normal volunteers retrospectively. Measured the ADC values and FF values of pelvic bones (pubis, iliac bone, sacrum and ischial bone) respectively, compared the diversity of the parameter values in two groups, and analyzed each parameter value in light, moderate and severe anemia groups and different anatomical sites. The Receiver Operating Characteristic(ROC) curve was used to evaluate the threshold value and the diagnostic efficiency of these parameter values. Results The ADC values and FF values in the normal group were higher than those in the anemia group. The area under the ROC curve was 0.843 and 0.838, and the diagnostic thresholds were 0.646×10-3mm2 / s, 65.47%, the ADC values andFF values in light anemia group were higher than those in moderate and severe anemia group(P<0.05). There was no significant difference in the parameters between the moderate and severe anemia groups, as well as the parameters in different anatomical sites in pelvis. Conclusion Magnetic resonance IDEAL-IQ and DWI imaging can indirectly reflect the anemia of women from the perspective of bone marrow component changes, and can preliminarily determine the degree of anemia.

前言
贫血是临床上常见的一类症状,其发生和发展不仅反映血液系 统异常,某些情况下还可以提示肿瘤、肾病等疾患的发生[1]。女性 由于月经、妊娠、哺乳、避孕等各类原因,成为贫血的高发人群。 以往利用影像学手段对贫血进行筛查,主要依靠胸部大血管或心 腔内血液CT值进行初步判定[2],但存在着大血管依赖性强、辐射较 高,难以反映骨髓造血功能变化等缺点。磁共振扩散加权成像技术 (diffusion weighted imaging,DWI)和水脂分离成像技术(iterative decomposition of water and fat with echo asymmetry and least square estimation-iron quantification,IDEAL-IQ)分别能够反映 组织内细胞增殖情况[3-4]及脂肪含量[5]。本研究旨在探讨同年龄段贫血 女性和正常女性DWI及IDEAL-IQ序列信号特征及相关参数值的差异,为 女性贫血的影像学诊断提供新的思路。
