双源CT双能量成像识别尿路结石成分的临床应用研究

摘要

目的探讨双源CT双能量成像技 术对体内尿石成分的识别价值。方法 选 择406例尿路结石患者共406枚结石进行 研究,均行西门子炫速双源CT(Siemens Somatom Definition Flash Dual-source CT,DSCT)进行双能量扫描并重建结石的彩 色编码图像;采用红外光谱分析确定所有 结石样本成分。结果 以红外光谱分析结 果为标准,双源CT双能量结石分析技术识 别尿酸类结石、胱氨酸结石、羟磷灰石、 草酸钙类结石及混合型结石成分的准确 性分别为96.7%,99.5%,98.5%,95.1%, 89.9%。结论 双源CT双能量技术能够有效 识别体内泌尿系结石的成分组成。

Objective To investigate the clinical value of the determination of renal stone composition in vivo with dual-source dual-energy computed tomography(DSDECT). Methods 406 cases with 406 renal stones were collected. All cases were scanned with dual-energy acquisition by dual-source CT. The data were sent to the workstation for the analysis of the stone composition. The urinary stones were analyzed by infrared spectroscopy after surgery. These two results were compared and the infrared spectroscopy result was taken as the reference standard. Results The accuracy of DSDECT for detecting the presence of four types of composition, uric acid, cysteine, hydroxyapatite, calcium oxalate and mixed stones, was 96.7%, 99.5%, 98.5%, 95.1% and 89.9% respectively. All cystine stones were with false positive results. Conclusion DSDECT could accurately predict the major component of urinary calculi and detect uric acid, cysteine, and calcium oxalate with a satisfactory accuracy.

前言

尿石症发病率逐年上升,为患者带来极大的痛苦;其治疗的手段 有保守药物治疗及手术。尿酸类结石可药物溶解,首选药物保守治 疗;羟磷灰石及草酸钙类结石不溶于药物,首选手术治疗。因此,治 疗前能明确体内结石成分,对治疗非常重要[1-3]。双源CT(dual source CT)近年发展成为辅助诊断的新方法,尤其在腹部病变诊断中具有巨大 优势[4]。利用双源CT双能量成分技术识别尿石成分的研究越来越多, 涉及大样本量的体内结石研究不多。本研究旨在以红外光谱分析结果 为参照,了解泌尿系结石在不同性别、年龄及发生部位的分布情况, 指导临床预防尿结的发生;并探讨双源CT双能量成像技术识别体内泌 尿系结石成分的可靠性。
