摘要
目的探讨乳腺癌术前应用MRI 动态增强成像联合扩散加权成像的诊断 价值。方法 选择2014年6月-2016年9月于 我院收治且经病理确诊为乳腺癌98例患者 临床资料,均行超声、DCE-MRI扫描、DWI 扫描,观察检查结果,比较各检查技术的 诊断符合率。结果 经手术病理证实,98 例患者中41例为浸润导管癌、16例为浸润 小叶癌、9例为导管内癌、32例为导管原 位癌。DCE-MRI+DWI诊断符合率92.86%比 超声64.29%、MRI常规扫描72.45%、DCEMRI 81.63%高(P<0.05),与DWI诊断符合 率86.73%无统计差异(P>0.05)。结论 乳 腺癌术前行MRI动态增强成像、扩散加权 成像联合诊断的价值高,可提高诊断符合 率,对手术治疗具指导意义。
Objective To observe the value of MRI dynamic enhanced imaging and diffusion weighted imaging in the preoperative diagnosis of breast cancer. Methods The clinical data of 98 patients with pathologically confirmed breast cancer admitted to our hospital between June 2014 and September 2016 were selected. All the patients were examined by ultrasonography, DCE-MRI scan, DWI scan, and the results were observed to compare the diagnostic coincidence rate of each inspection technique. Results Confirmed by surgery and pathology, in 98 cases of patients, 41 cases of invasive ductal carcinoma, 16 cases of invasive lobular carcinoma, 9 cases of intraductal carcinoma and 32 cases of ductal carcinoma in situ. DCE-MRI + DWI diagnostic coincidence rate was 92.86%, which was higher than 64.29% of ultrasonography, 72.45% of MRI routine scan and 81.63% of DCE-MRI (P<0.05), and had no statistical difference with the diagnostic coincidence rate of DWI 86.73% (P>0.05). Conclusion The value of MRI dynamic enhanced imaging and diffusion weighted imaging in the preoperative diagnosis of breast cancer is high, which can improve the diagnostic coincidence rate, and has guiding significance for surgical treatment.

前言
乳腺癌在女性恶性肿瘤中患病率极高,发病率呈逐年增加且趋于 年轻化,及早诊治可改善病情预后,提高生存率。B超为以往临床诊断 乳腺癌常用的影像学方法,灵敏性、特异性均较高,但其检出率易受 检查者水平影响[1]。近年,磁共振技术已成为乳腺癌诊断新技术,磁 共振扫描(MRI)与扩散加权成像(DWI)被广泛用于乳腺癌诊断中,其中 MRI识别乳腺癌的分辨率高,更能凸显肿瘤的血流动力学、形态特点, 且可显示微小肿瘤,可提升诊断准确性[2-3]。另外,MRI动态增强扫描 (DCE-MRI)可利用血流动力学分析病变,了解病灶形态,以判定病变性 质[4]。因此,我院联合MRI动态增强、扩散加权的成像技术为近期收治 的98例乳腺癌患者行术前诊断,旨在为手术提供更全面的参考依据, 现报道如下。
