摘要
目的 研究胰腺神经内分泌肿瘤 (PNETs)的CT表现以及CT在其良、恶性及 不同类型鉴别中的应用价值。方法 选取 我院73例PNETs患者为研究对象,均经手 术病理及穿刺活检确诊为PNETs,接受CT 平扫及增强扫描,观察PNETs的CT表现, 测算出良性、恶性PNETs的CT值差异,并 对功能性PNETs与无功能性PNETs的CT差异 进行分析。结果 恶性患者肿瘤直径1.2- 11.7cm,良性患者肿瘤直径0.4-4.8cm, 两者比较恶性PNETs患者瘤灶直径明显较 大(P<0.05),良性形态不规则,多处于 胰头处,瘤灶内部结构实性25例,剩余 6例均为囊实性,无囊性、钙化及胰胆管 扩张。恶性瘤灶部位多处于胰头及胰体尾 处,囊实性37例,均有钙化出现,胰胆 管扩张者37例,明显高于良性患者;良 恶性PNETs平扫时CT值无统计学意义(P> 0.05),而动、静脉及平衡期良性PNETs患 者CT值均高于恶性PNETs患者,组间比较 有统计学意义(P<0.05);功能性PNETs共 11例,其中良性9例,恶性2例,无功能性 PNETs共62例,恶性40例,良性22例,无 功能性PNETs病灶直径2.7-11.7cm,功能 性PNETs 0.4-7.9cm。结论 PNETs有其特 殊的CT征象,CT增强扫描可实现良性、恶 性PNETs以及功能性、非功能性PNETs的鉴 别,在治疗方案选择、病情判断、预后评 估中有重要的应用价值。
Objective To study the CT findings of pancreatic neuroendocrine tumors (PNETs) and the application value of CT in the differentiation of benign, malignant and different types of the tumors. Methods 73 cases of patients with PNETs in our hospital were selected as the research objects and all PNETs were confirmed by surgical pathology and biopsy. All patients underwent CT plain scan and enhanced scan. CT findings of PNETs were observed. The differences in CT values between benign and malignant PNETs were measured and the differences of CT in functional PNETs and nonfunctional PNETs were analyzed. Results The diameter of malignant tumors was 1.2- 11.7cm, while of benign ones was 0.4-4.8cm. The diameter of the malignant tumors was significantly larger than that of benign ones (P<0.05). The shape of benign tumors was irregular and most of them were located in the head of pancreas. The internal structures of 25 cases of tumor lesions were solid and the remaining 6 cases were cystic solid, without cystic, calcification and dilatation of bile duct. Most of the sites of malignant tumors were the head of pancreas and pancreatic body and tail. There was calcification 37 cases which were cystic solid and 37 cases with dilatation of pancreatic duct, significantly higher than those in patients with benign tumors; There was no statistical significance in CT values of benign and malignant PNETs when plain scan (P>0.05) while CT values in patients with benign PNETs in arterial, venous and equilibrium phase were higher than those in patients with malignant PNETs (P<0.05). Functional PNETs were in 11 cases, including 9 cases of benign and 2 cases of malignant ones. There were 62 cases of non functional PNETs, including 40 cases of malignant and 22 cases of benign ones. The diameter of non-functional PNETs lesions was 2.7-11.7cm while of functional PNETs was 0.4- 7.9cm. Conclusion PNETs has its special CT findings. CT enhanced scan can identify benign and malignant PNETs and functional and non functional PNETs, which has important application value in the selection of treatment, judgement of pathogenetic condition and evaluation of prognosis .

前言
胰腺神经内分泌肿瘤(pancreatic neuroendocrine tumors, PNETs)为胰腺多能神经内分泌干细胞,与胰腺癌相比,均可由良性发 展至恶性,病程较长,且总体发病率较低,占胰腺肿瘤的1%~3%[1], 同时症状表现多样化,临床多误诊为内分泌疾病,由临床表现可分为 功能性PNETs及无功能性PNETs,无功能性PNETs的肿瘤激素分泌少,无 特异性临床表现,因此容易漏诊[2]。PNETs诊断方式包括CT、MRI、内 镜超声等,本次研究围绕我院73例PNETs患者的CT诊断结果进行分析, 旨在观察CT用于PNETs诊断的准确性以及CT对PNETs不同类型及良、恶 性鉴别中的应用,现将结果报告如下。
