摘要
目的 应用320CT的运动成像功 能来评判类风湿关节炎受试者的关节运 动及其与类风湿因子的相关性。方法 选 择类风湿性关节炎且累及其双侧腕关节 者20例,应用320层动态CT运动成像研究 其腕关节,检测尺侧的偏转度、桡侧的偏 转度、尺桡侧的活动度以及血清中类风 湿因子的水平。结果 受试者双侧腕关节 的尺侧角度的配对检验结果:P=0.494; 受试者桡侧的角度配对的t检验结果: P=0.309;受试者尺桡活动度的配对t检验 结果:P=0.187;受试者尺侧偏转度和血 清中的类风湿因子水平的相关性检验: r=-0.373,P=0.021;受试者桡侧的偏转 度和血清中的类风湿因子的相关性检验: r=-0.509,P=0.003;受试者尺桡侧的活 动度与血清中的类风湿因子的相关性检 验:r=-0.440,P=0.007。结论 类风湿性 关节炎患者双侧腕关节的运动障碍程度无 统差异,患者腕关节的运动功能和类风湿 因子呈负相关。
Objective To judge the joint movement of the subjects of rheumatoid arthritis and its correlation with rheumatoid factor though the application of 320CT motion imaging function. Methods Choice 20 cases of rheumatoid arthritis, and bilateral wrist joint. Study of the wrist joint, and measure the deflection of the ulnar side, the deflection of the radial side, the activity of the ulnar radial side, and the level of serum rheumatoid factor though the application of 320CT motion imaging function. Results Paired test results of the ulnar side of the wrist of the participants: P=0.494. T test results from the angle of the radial side of the receiver: P=0.309. Paired t test results of the ulnar radial activity of the subjects: P=0.187. Correlation test between the ulnar deflection and the serum levels of rheumatoid factor: r=-0.373, P=0.021. The correlation between the radial side of the test and the test of rheumatoid factor in the radial side of the test: r=- 0.509, P=0.003. The correlation between the level of activity of the ulnar radial side of the test and the rheumatoid factor in serum: r=-0.440, P=0.007. Conclusion There was no difference in the degree of the movement of the wrist in the patients with rheumatoid arthritis, and there was a negative correlation between the motor function of the wrist and the rheumatoid factor.

前言
类风湿性关节炎属于全身自身免疫疾病,其的发病率大约1%,成 年女性多发,多数患者血清中的类风湿因子水平增高[1]。其特征性改 变是小关节对称损伤,如腕关节的关节滑膜增厚,韧带、软骨损伤、 关节晨僵。东芝公司320层动态CT,能够在容积扫描的基础上融入时间 轴,最终能够获取实时动态影像[2]。应用该320层动态CT的扫描模式, 能够使用不同时相序列进行扫描,可实时研究患者小关节对称损伤情 况。本研究应用320层 动态CT对20名类风湿性关节炎受试者的腕关节 进行运动成像,并且测量受试者血清中的类风湿因子水平,分析受试 者腕关节运动能力与血清类风湿因子水平的线性相关性。现报道如 下。
