探究不同剂量CT扫描技术在臀部窦道造影患者中的应用 价值

目的 比较不同剂量CT扫描技术 在臀部窦道造影的图像质量及应用价值。 方法 收集我院创疡科2013年3月-2015年 3月收治怀疑异物致臀部窦道造影患者110 例作为研究对象,随机分为常规组(扫描 剂量200mA,30例)与低剂量A组(扫描剂 量100mA,38例)及低剂量B组(扫描剂量 50mA,42例)。比较三组的ED、CTDIvol、 DLP并进行图像质量评价。结果 三组DLP 之间的两两比较无统计学意义(P﹥0.05)。 低剂量A组ED、CTDIvol分别为(7.4±0.7) mSv、(8.6±0.8)mGy,低剂量B组的 ED、CTDIvol分别为(4.2±0.4)mSv、 (4.3±0.5)mGy,低于常规组(16.2±1.1) mSv、(18.9±0.6)mGy,差异有统计学 意义(P﹤0.05)。常规组图像质量优良率 为100%,低剂量A组为92.1%,与常规 组接近;低剂量B组图像质量优良率为 83.3%,与常规组比较,有统计学意义(P <0.05)。结论 臀部窦道的诊断对图像质 量要求不高,管电流50 mA与常规剂量相 比可减少辐射损伤,同时也满足诊断需 求。
Objective To compared the image quality and application value of different doses of CT scanning technique in hip contrast fistulography. Methods 110 cases of patients who were suspected of having foreign bodies and underwent hip contrast fistulography in the department of traumatology of our hospital between March 2013 and March 2015 were selected as the study objects and were randomly divided into the routine group (scanning dose was 200 mA, 30 cases) and the low dose group A (scanning dose was 100 mA, 38 cases) and low dose group B (scanning dose was 50 mA, 42 cases). ED, CTDIvol and DLP were compared between the three groups. The image quality was evaluated. Results There were no significant differences in DLP between any two groups of the three groups (P﹥0.05). ED and CTDIvol in the low dose group A [(7.4±0.7) mSv, (8.6±0.8) mGy] and low dose group B [(4.2±0.4) mSv, (4.3±0.5) mGy] were lower than those in the routine group [(16.2±1.1) mSv, (18.9±0.6)mGy] (P<0.05). The excellent and good rates of image quality in the routine group (100%) and the low dose group A (92.1%) were close to that in the routine group. The excellent and good rate of image quality in the low dose group B was 83.3%. Compared with that in the routine group, there was significant difference (P<0.05). Conclusions The requirement of image quality in diagnosis of buttock sinus is not high. Compared with routine dose, tube current of 50 mA can reduce the radiation damage and also meets diagnostic needs.

前言
窦道指组织坏死后所形成只开口于皮肤黏膜表面的深在性盲管, 不与体内空腔脏器相沟通,多为感染后引流不畅或异物遗留所致。臀 部CT扫描临近人体生殖器官,X线辐射剂量越大其辐射损伤越大,易 引起精子或卵子畸变[1],后果严重,因而CT扫描的低剂量研究非常必 要。同时,脂肪对CT扫描结果有一定影响,而臀部脂肪较多,因而有 关减少辐射剂量同时确保图像清晰度的研究也同样重要[2]。本研究以 我院收治怀疑异物致臀部窦道造影患者为例,比较不同剂量CT扫描技 术在臀部窦道造影的图像质量及应用价值,现报道如下。
