MRI与钼靶X线对乳 腺癌术前瘤体体积 及病理分型的评估 价值

摘要

目的 探讨磁共振成像(MRI)与 钼靶X线对乳腺癌术前瘤体体积及病理分 型的评估价值。方法 选择医院2016年4 月-2018年4月收治的乳腺癌患者120例, 所有患者均经手术病理证实为乳腺癌。术 前均接受乳腺MRI、钼靶X线检查,比较两 种检查方式的检出率、瘤体体积及病理分 型情况。结果 MRI在乳腺癌的检出率高于 钼靶X线检查(P<0.05),对瘤体体积评估 与病理结果的一致性高于钼靶X线检查(P <0.05),对于导管原位癌、浸润性小叶 癌及粘液腺癌病理分型的符合率高于钼靶 X线检查(P<0.05)。结论 MRI在术前乳腺 癌的检出率高于钼靶X线,病例分型也较 准确,对瘤体体积的评估与病理结果一致 性较高。

Objective To explore the evaluated value of magnetic resonance imaging (MRI) and mammography in the preoperative tumor volume and pathological types of breast cancer. Methods 120 cases of patients with breast cancer who were admitted to the hospital from April 2016 to April 2018 were selected. All patients were confirmed to be breast cancer by surgical pathology. All patients were given breast MRI and mammography before operation. The detection rate, tumor volume and pathological types of the two examination methods were compared. Results The detection rate of breast cancer by MRI was higher than that by mammography (P<0.05), and the consistency of tumor volume assessment and pathological results was higher than that of mammography (P<0.05), and the coincidence rates of pathological types of ductal carcinoma in situ, invasive lobular carcinoma and mucinous adenocarcinoma were higher than those by mammography (P<0.05). Conclusion The detection rate of preoperative breast cancer by MRI is higher than that by mammography, and MRI has accurate pathological types, and has high consistency of tumor volume assessment and pathological results.

前言

乳腺癌的发病率占女性全身各种恶性肿瘤的7%以上,是女性最易 发的恶性肿瘤之一,严重影响了女性的生理、心理健康,早期诊断及 时治疗是改善预后的关键[1]。钼靶X线主要通过乳腺组织间的密度差显 示其整体形态及病灶特点,对于其典型钙化的特异性较高,但其无法 提供癌灶的血供情况,对于不典型的病灶则极易漏诊,对乳腺癌的检 出率及肿瘤范围的准确评估均不能达到预期。近年来,动态增强乳腺 磁共振成像(MRI)可较准确的评估乳腺癌肿瘤大小及范围,临床应用于 检查乳腺癌也越来越普遍。大量研究表明,MRI对乳腺癌的检出率超过 90%,是评价乳腺病变的有效检查方法[2-4]。目前MRI与钼靶X线的比较 研究国内报道还较少。本研究通过与手术病理进行对照,探讨MRI与钼 靶X线对乳腺癌术前瘤体体积及病理分型评估的价值,旨在为提高乳腺 癌患者术前检查准确性提供依据。现报道如下。
