多层螺旋CT三维重 建技术与X线影像 学检查在盆骨骨折 诊治中应用比较*

目的 多层螺旋CT三维重建技 术与X线影像学检查在骨盆骨折诊治中 应用比较。方法 选取我院2014年7月至 2016年7月70例骨盆骨折患者为研究对 象,所有患者均给予多层螺旋CT三维重 建与X线影像学检查,以病理学诊断结 果为准,比较两种影像学检查对骨盆骨 折的诊断结果。结果 70例患者共有113 处骨折,多层螺旋CT三维重建对各种部 位骨折的诊断率均为100%,X线片对髋臼 后唇、后柱及骶髂关节骨折漏诊5例,髋 臼中间壁骨折漏诊4例,髋臼顶壁、骶尾 骨、髂骨翼骨折漏诊各3例,诊断准确率 为79.65%。多层螺旋CT三维重建与X线检 查对髋臼前唇、前柱,髋臼顶壁,耻骨 上、下支,坐骨上、下支,骶尾骨,髂 骨翼,耻骨联合分离诊断率比较无统计 学意义(P>0.05);多层螺旋CT三维重 建对髋臼后唇、后柱,髋臼中间壁及总 骨折诊断率均高于X线片,有统计学意义 (P<0.05)。结论 多层螺旋CT三维重建 可明确诊断各种类型的骨盆骨折,诊断 率非常高,还可全面、精确地反映出骨 折部位的空间构象及骨盆的立体形态, 具有较好的临床应用价值。
Objective To compare the application of multi-slice spiral CT threedimensional reconstruction and X-ray in the diagnosis and treatment of pelvic fracture. Methods Seventy patients with pelvic fracture admitted to our hospital between May 2013 and May 2015 were examined by multi-slice spiral CT three-dimensional reconstruction and X-ray. According to the results of pathological examination, the two kinds of imaging examinations were compared in diagnosis of pelvic fracture. Results There were 113 fractures in 70 patients. The diagnostic rate of multi-slice spiral CT three-dimensional reconstruction for fractures of various parts was 100%. There were 5 cases with acetabular posterior lip, posterior column and sacroiliac fractures, 4 cases with acetabular midfeather fractures and 3 cases with acetabular roof, sacrococcyx and sacrum and iliac pterygoid bone fractures never diagnosed by X-ray. The diagnostic accuracy was 79.65%. There were no significant differences between multi-slice spiral CT three-dimensional reconstruction and X-ray in single and combined diagnostic rate of acetabular anterior lip, anterior column, acetabular roof, upper and lower branch of pubis, upper and lower branch of ischium, sacrococcyx, iliac pterygoid bone and pubis (P>0.05). The diagnostic rates of multislice spiral CT three-dimensional reconstruction for acetabular posterior lip and posterior column, acetabular midfeather and total fracture diagnostic rate were higher than those of X-ray (P<0.05). Conclusion Multislice spiral CT three-dimensional reconstruction can be used to diagnose various types of pelvic fractures, and the diagnostic rate is very high. It also can comprehensively and precisely reflect the stereo configuration of the fractured parts and pelvic three-dimensional shape.

前言
骨盆属于不规则骨,解剖结构复杂,主要由双侧髋骨、骶骨、尾 骨及其间的骨连接构成的环状骨性结构[1]。骨盆骨折是一种复杂的骨 折类型,主要由交通事故、高处坠落、重物砸压等高能量损伤造成, 约占全身骨折的10%~22.1%[2]。骨盆骨折易引起严重的出血、腹腔重 要脏器损伤休克等多种并发症[3],致残率高达4.8%~50.0%[4]。传统多 采用X线平片进行诊断,随着医学影像学的发展,多层螺旋CT及三维重 建技术开始广泛用于临床[5]。本研究主要探讨了多层螺旋CT三维重建 技术与X线影像学检查在骨盆骨折诊治中应用比较,选取我院2014年7 月至2016年7月70例骨盆骨折患者为研究对象进行研究,现将结果报道 如下。
