超声/CT融合成像 对肾肿瘤诊断价值 研究

目的 探讨超声/CT融合成像在 鉴别肾肿瘤中的应用价值。方法 回顾 性分析2014年6月-2016年6月经临床病 理确诊为肾肿瘤患者79例的超声、CT 影像学资料,并行超声/CT融合成像分 析。结果 肾恶性肿瘤57例,超声图像 上多为低回声或等回声,CDFI显示53例 肿瘤内部有彩色血流信号显示,4例肿 瘤周围有细条状血流;CT平扫以低密度 为主,多数增强扫描示为高增强或等增 强。良性肿瘤22例,超声图像上多为高 回声,多数CDFI无血流信号显示,仅4例 肿瘤周边有细条状血流;CT平扫以低密 度为主,多数增强为低增强或等增强。 超声/CT融合、CT、超声诊断肾肿瘤准 确性(96.20%,84.81%,70.89%)、敏感 性(96.49%,85.96%,73.68%)、特异性 (95.45%,81.82%,63.64%)比较,差异 有统计学意义(P<0.05)。结论 超声/CT 融合成像技术综合了超声、CT的优势, 可提高肾肿瘤的定性诊断准确率,有利 于指导临床确定治疗方案。
Objective To explore the value of ultrasound/CT fusion imaging in the differential diagnosis of renal tumors. Methods The ultrasonic and CT imaging data of 79 patients with pathologically confirmed renal tumors diagnosed between June 2014 and June 2016 were analyzed retrospectively. The ultrasound/CT fusion imaging analysis was performed. Results There were 57 cases of renal malignant tumors, and most of them showed low echo or equal echo on ultrasound images. CDFI showed that there were color blood flow signals in the tumors in 53 cases and fine blood flow around the tumors in 4 cases. CT scan mainly showed low density, and the enhanced scan mainly showed high or equal enhancement. There were 22 cases of benign tumors, and most of them showed high echo on ultrasound images. CDFI showed no blood flow signals in most of them, and there were only 4 cases with fine blood flow around the tumors. CT scan mainly showed low density, and most of them showed low or equal enhancement. There were significant differences among ultrasound/CT fusion, CT and ultrasound in accuracy rates (96.20%, 84.81%, 70.89%), sensitivities (96.49%, 85.96%, 73.68%) and specificities (95.45%, 81.82%, 63.64%) in the diagnosis of renal tumors (P<0.05). Conclusion Ultrasound/CT fusion imaging integrates the advantages of ultrasound and CT scan. It can improve the accuracy of qualitative diagnosis of renal tumors, and it is helpful to guide clinical treatment.

前言
肾肿瘤是临床常见的泌尿外科较为常见的肿瘤,其中80%以上为 恶性,以肾透明细胞癌最为常见,恶性程度最高,且发病率呈逐年增 加[1]。肾良性肿瘤中肾错构瘤发病率较高,无明显症状,多为体检偶 然发现。临床研究发现,肾肿瘤发病期间无特异性症状,增加了鉴别 诊断难度。超声、CT等是临床诊断肾肿瘤较为常用的影像学方式,但 在疾病定性诊断、鉴别诊断中均存在局限性。融合成像技术主要将几 种影像学图像经处理后达到空间坐标上的匹配,叠加后获取互补信 息[2]。目前,融合成像技术已用于肿瘤诊断及物理治疗、图像引导手 术的导航系统中[3]。然而,超声/CT融合技术用于肾肿瘤鉴别诊断的研 究报道还相对较少。对此,本研究分析超声、CT、超声/CT融合成像在 鉴别诊断肾肿瘤良、恶性中的应用情况,并与临床病理对照,以期提 高肾肿瘤定性诊断准确性,指导临床确定手术方案,现报告如下。
