目的 研究超声联合3.0MRI(磁 共振成像技术)在剖宫产术后切口瘢痕 妊娠中诊断价值分析。方法 选取我院 2011年1月至2016年1月200例临床怀疑为 剖宫产术后切口瘢痕妊娠患者为研究对 象，所有患者均在术前进行超声检查， 对超声检查存疑的患者再行MRI检查。以 手术病理结果为金标准，计算超声联合 3.0MRI诊断的灵敏度、特异度、阳性预 测值、阴性预测值、准确性。结果 超 声联合MRI检查的灵敏度、特异度、准 确率、Kappa值分别为98.7%、94.2%、 97.5%、0.93。联合检查的特异度和准确 率显著高于超声检查的71.2%、89.5%(P ＜0.05)。结论 超声联合3.0MRI可以提 高剖宫产术后切口瘢痕妊娠诊断的准确 率，为治疗方案的选择提供可靠的影像 学依据。

Objective To study the value of ultrasound combined with 3.0MRI (magnetic resonance imaging) in the diagnosis of cesarean scar pregnancy after cesarean section. Methods 200 patients with suspected cesarean scar pregnancy after cesarean section admitted in our hospital between January 2011 and January 2016 were selected as study subjects. All patients underwent ultrasonography before operation, and suspicious patients underwent MRI after ultrasonography. With surgical and pathological results as the golden standard, the diagnostic sensitivity, specificity, positive predictive value, negative predictive value and accuracy of ultrasound combined with 3.0MRI were calculated. Results The sensitivity, specificity, accuracy and Kappa value of ultrasound combined with MRI were 98.7%, 94.2%, 97.5% and 0.93, respectively.The specificity and accuracy of combined examination were significantly higher than those of ultrasound (71.2%, 89.5%) (P<0.05). Conclusion Ultrasound combined with 3.0MRI can improve the accuracy in the diagnosis of cesarean scar pregnancy after cesarean section, and provide reliable imaging evidence for the choice of treatment plan.
