MSCT重建技术在腕关节骨折诊治中应用研究

摘要

目的 探讨多层螺旋CT(MSCT) 在腕关节骨折诊断中的应用价值。方 法 收集2012年2月-2015年7月我院收治 的60例腕关节骨折患者,所有患者入院 后均接受X线、MSCT检查,配合重建技术 处理,且临床资料完整,分析MSCT重建 技术对腕关节骨折诊断的临床价值。结 果 MSCT诊断腕关节骨折符合率高于X线 片(P<0.05);MPR重建、VR重建检出腕 关节骨折准确率相近,均高于CT平扫(P <0.05);MSCT对关节脱位、骨折、移位 骨碎片及骨挫伤诊断准确率均高于X线片 (P<0.05)。结论 MSCT重建技术可直观、 立体显示腕关节骨折征象,提高腕关节 骨折诊断准确率,为临床治疗提供影像 学依据。

Objective To investigate the application value of multi-slice spiral CT (MSCT) in the diagnosis of wrist fractures. Methods Sixty patients with wrist fractures treated in our hospital between February 2012 and July 2015 were enrolled in the study. All patients underwent X-ray and MSCT examination combined with reconstruction technique treatment after admission. The clinical data were complete. The clinical value of MSCT reconstruction technique in the diagnosis of wrist fractures was analyzed. Results The coincidence rate of MSCT in the diagnosis of wrist fractures was higher than that of X-ray film (P<0.05). The accuracy rates of MPR and VR were close in detection of wrist fractures, which were higher than that of CT plain scan (P<0.05). The accuracy rates of MSCT in the diagnosis of joint dislocation, fractures, displaced bone fragments and bone contusion were higher than those of X-ray film (P<0.05). Conclusion MSCT reconstruction technique can directly and stereoscopically display the signs of wrist fractures, improve the diagnostic accuracy, and provide basis for clinical treatment.

前言

腕关节骨折为临床常见骨折类型,中老年群体常见,多由跌倒后  手掌硬性支撑、着地引起 。且人体腕关节由月骨、舟骨、三角骨等多骨构成,骨骼解剖结构复杂,形态非规则,排列紧密但体积小,临 [2] 床诊断困难 。若未及时进行临床干预,可能导致腕关节畸形,引起 关节活动受限,影响患者生活质量。而早期确诊腕关节骨折类型,可 为临床治疗提供依据,促进腕关节骨折恢复。以往常采用X线片诊断腕  关节骨折,但其对部分隐匿性骨折检出率低,同时易受到邻近骨骼遮  蔽的影响,假阳性率较高[3]。多层螺旋CT(MSCT)则有其较高的组织分 辨率,可为腕关节骨折提供更为丰富的诊断信息,同时配合三维重建 后处理技术,可多角度、多平面观察骨折形态变化,有助于明确骨折相关解剖关系,对细微骨折、隐匿性骨折均有较好的显示率[4]。基于此,为探讨MSCT重建技术在腕关节骨折诊断中的应用价值,我院对收治的60例患者的临床资料展开了回顾性分析,现报道如下。
