64层螺旋CT血管造影在腹部中小动脉粥样硬化诊断中的 应用

摘要

目的探讨64层螺旋CT血管造影 (CTA)在腹部中小动脉粥样硬化诊断中的 应用价值。方法 回顾性分析52例腹部中 小动脉粥样硬化患者临床资料,均行64层 螺旋CTA检查,在容积再现(VR)、最大密 度投影(MIP)等后处理技术基础上进行血 管探针技术(VP)处理,分析检查结果。结 果 本组52例患者中,47例腹主动脉及各 分支发现动脉粥样斑块,占90.38%,共 检出斑块119处;其中31例腹主动脉1级 分支,10例腹主动脉2级分支,6例出现腹 主动脉3级分支。VP表现为血管壁明显增 厚轮廓欠清晰,且较为模糊;管腔充盈缺 损,有不同程度的狭窄;管壁有不同程度 的钙化影,动脉瘤显示局部动脉管腔向外 扩张;部分血管走形僵直,血管僵硬。结 论 64层CTA是诊断腹部中小动脉粥样硬化 的有效方法,对临床诊治具有指导意义。

Objective To investigate the application value of 64 slice spiral CT angiography (CTA) in the diagnosis of abdominal small and medium atherosclerosis. Methods The clinical data of 52 patients with abdominal small and medium atherosclerosis were retrospectively analyzed. All patients underwent 64 slice spiral CTA. Based on volume rendering (VR), maximum intensity projection (MIP) and other post processing techniques, vessel probe (VP) was performed and the results were analyzed. Results In this group of 52 patients, arterial atherosclerotic plaques were found in 47 cases of abdominal aorta and branches, accounting for 90.38% and a total of 119 lesions were detected, including 31 cases of abdominal aortic grade 1 branches, 10 cases of abdominal aortic grade 2 branches and 6 cases of abdominal aortic grade 3 branches. VP showed markedly thickened vascular walls with blurry outlines, luminal filling defect with different degrees of stenosis; There were different degrees of calcified shadows. Aneurysm displayed outward expansion of the arterial lumen; Part of the blood vessels were tetanic and stiff. Conclusion 64 slice CTA is an effective method for diagnosis of abdominal small and medium atherosclerosis, which is of guiding significance for clinical diagnosis and treatment

前言

近年来,临床腹部血管性疾病发病率明显上升,肾动脉、长动脉 等中小动脉均可发生血管性病变,严重影响着患者生活质量。数字减 影血管造影技术(DSA)是临床诊断血管病变的“金标准”,但属于有创 检查,且价格较为昂贵,临床应用受限。随着CT技术发展,多层螺旋 CT逐渐用于血管疾病诊断中,尤其是多层螺旋CT血管造影(MSCTA),具 有强大图像后处理技术,能够提高空间分辨率,清楚显示腹部血管病 变情况、病变程度及与周围组织的关系,在临床确定治疗方案、评估 疗效中具有较高的应用价值[1]。对此,本文分析了64层螺旋CTA VP技 术在腹部中小动脉粥样硬化中的应用情况,探讨MSCTA在诊断腹部病变 中的应用价值,现报道如下。
