磁敏感加权成像在高血压伴发脑内微出血中诊疗价值探析

摘要

【摘要】目的  探讨磁敏感加权成像(SWI)在高血压伴发脑内微出血(CMBs)中的诊疗价值。方法  入选我院2011年8月至2014年8月收治的44例高血压患者,进行常规及SWI序列扫描,比较各序列对CMBs灶的数目、部位和分布情况,以及高血压水平与CMBs的发生关系。结果  经检查,高血压伴有CMBs的患者比例为50.0%(22/44),且血压显著高于无CMBs组患者;SWI检测出的CMBs病灶数目682个显著高于T2WI  85个和T2FLAIR  136个,差异具有统计学意义(P<0.05);且CMBs在基底节区分布最多31.7%(216/682),其次为皮层-皮层下,在小脑分布最少。结论  SWI在高血压伴发CMBs中具有明确的诊疗价值。

Objective To investigate the susceptibility weighted imaging (SWI) in hypertension with cerebral microbleeds (CMBs) in the diagnosis and treatmentvalue. Methods 44 patients with essential hypertension were selected in our hospital from 2012 August to 2013 August were, routine and SWI sequencescanning, condition number, location and distribution of CMBs foci compared with various sequences, and the level of blood pressure and the occurrence of CMBs relationship. Results after inspection, the hypertension patients with CMBs ratio of 50.0%(22/44), and blood pressure was significantly higher than that in non CMBs group patients; the number of CMBs lesions were detected by the SWI 682 was significantly higher than that of T2WI 85and T2FLAIR136 with significant difference (P<0.05); and CMBs in the basal ganglialocated at 31.7%(216/682), followed by cortico subcortical and cerebellardistribution, at least. Conclusion SWI has the value of diagnosis and treatmentin hypertension with clear CMBs.

前言

脑微出血(Cerebral microbleeds,CMBs)是微小血管病变导致的 含铁血黄素沉积[1],常见于老年患者,与老龄、高血压和心脏疾病相 关。CMBs临床症状隐匿,活体检查较难发现。由于去氧血红蛋白磁敏 感性较强,CMBs在梯度回波磁共振成像(MRI)上表现为脑内低信号病 灶。但CMBs病灶一般<5mm[2],常规MRI扫描较难发现。磁敏感加权成 像(susceptibility weighted imaging,SWI)作为一种能够反映组织 间内在磁敏感特性差别的新型成像技术,易于检出造成局部磁场不均 匀性的病变 ,尤其对出血和出血产物的显示敏感性更高。因此,SWI 在颅脑疾病中的应用愈加广泛。我院特入选44例高血压患者,采用SWI 检测CMBs,效果满意,现报告如下。
