摘要
目的探究第二代双源CT低剂量 法全肝灌注在原发性肝癌的肝动脉化疗栓 塞术(TACE)前后的血供情况。方法 选取 原发性肝癌并采用TACE术治疗的患者共32 人,其中男性患者27人,女性患者5人,年龄 31-76岁。两组患者分别采用kV值、mAs值 不同的扫描。术前7天及术后20天分别进 行三期常规扫描和灌注扫描。把甲、乙组 灌注扫描辐射剂量分别与肝脏常规三期扫 描、水模扫描辐射剂量进行对比分析。结 果 两组患者栓塞术前后双源CT低剂量灌 注扫描效果良好,获取了准确的扫描数据 和清晰的容积图像。两组常规肝脏增强扫 描与双源CT低剂量灌注扫描采用单样本t 检验,甲:P<0.05; 乙:P>0.05。乙组 辐射剂较甲组低,相对于常规平扫加肝脏 增强扫描需要的辐射剂量更为接近。结论 第二代双源CT大幅度地降低了扫描的kV 值、mAs值,可以捕获较为清晰的肝脏低剂 量灌注图像,并且第二代双源CT低剂量法 全肝灌注在原发性肝癌的TACE术治疗前后 的血供情况观察效果良好,值得临床广泛 使用。
Objective To explore the second-generation dual-source CT low-dose whole liver perfusion method in the blood supply of primary liver cancer hepatic arterial chemoembolization (TACE) before and after. Methods Primary liver cancer and the use of TACE treatment of patients with a total of 32 people, including 27 males, 5 females, aged 31 to 76 years. Two groups of patients were using the kV, mAs values of different scans. Preoperative 7 days and 20 days respectively after three regular scans and perfusion scan. The A, B perfusion scan radiation doses were associated with liver conventional three scanning, water phantom scan radiation dose were analyzed. Results The two groups of patients before and after embolization of low-dose dual-source CT perfusion scan to good effect, to obtain an accurate scan data and a clear volumetric image. Two conventional liver enhanced scan and dual source CT perfusion scan using a single low-dose sample t-test, A: P <0.05; B: P> 0.05. Conclusion The second-generation dual-source CT significantly reduce the value kV, mAs value scanning, you can capture a clearer picture of liver low-dose infusion, and the second-generation dual-source CT low-dose whole liver perfusion method in primary liver cancer the TACE technique to observe the effect of the blood supply before and after treatment is good, worthy of widespread use.

前言
德国西门子公司研制出一种新型CT(双源计算机断层扫描)扫描仪 Somatom Definition Flash dual-source CT,只需要原来扫描所需 辐射剂量的一小部分扫描出最小的细节。该系统扫描速度非常快大约 43cm/s,时间分辨率为75ms,所以完整扫描整个胸部范围仅需0.6秒。 因此,医生可以不要求他们的病人在检查期间摒住呼吸。TACE术是指 将导管选择性或超选择性插入到肿瘤供血靶动脉后,以适当的速度注 入适量的栓塞剂,使靶动脉闭塞,引起肿瘤组织的缺血坏死。目前最 多用于肝癌的治疗。CT灌注成像不同于动态扫描, 可以量化反映局部 组织血流灌注量的改变,因此对明确病灶的血液供应具有重要意义。
