Application of Multi-slice Spiral CT in the Diagnosis and Prognosis Evaluation of Neonatal Hypoxic Ischemic Encephalopathy

Abstract

Objective To explore the application value of multi-slice spiral computed

tomography (MSCT) in the diagnosis and prognosis evaluation of neonatal hypoxic ischemic encephalopathy (HIE). Methods The clinical data of 90 children patients with HIE admitted between June 2016 and January 2018 were retrospectively analyzed. All patients were examined by MSCT. The HIE grades diagnosed by MSET were evaluated,and the clinical manifestation grading was used as the gold standard to evaluate the grades efficacy of MSCT. The relationship between MSCT manifestation and developmental quotient (DQ) at 3 months and 6 months of follow-up after birth was evaluated. Results Among the 90 patients with HIE diagnosed by MSCT, there were 45 mild cases, 32 moderate cases and 13 severe cases. With clinical grading as the gold standard, theaccuracy rates of MSIE in the diagnosis of mild, moderate and severe HIE were 86.67%, 85.56% and 94.44%respectively. MSCT images showed that there were 45 cases of low-density lesions <2 brain lobes and 45 cases of≥ 2 brain lobes and above among the 90 children patients, and there were 11 cases of intracranial multiple site hemorrhage, 41 cases of simple subarachnoid hemorrhage, 24 cases of external hydrocephalus, 7 cases of basal ganglia lesions (including 4 cases of reversal sign) and 8 cases of ventricular enlargement. 3 months of follow-up after birth showed that there was 26 cases of children patients with DQ<90 (28.89%, 26/90) among the 90 children patients. 6 months of follow-up after birth showed that there were 14 children patients with DQ<90 (15.56%, 14/90) among the 90 children patients. At 3 months and 6 months after birth, the proportions of HIEchildren patients with DQ<90 with low-density lesions≥2 cerebral lobes, intracranial hemorrhage, basal ganglia lesions and ventricular enlargement were higher (P<0.05), and there was no significant difference in the proportion of external hydrocephalus between DQ<90 patients and DQ≥90 patients (P>0.05). Conclusion MSCT examination has good efficacy in diagnosing HIE and is useful for providing information for predicting prognosis in children patients.
