术前MRI在评估不同 质地垂体瘤中的应 用

摘要

目的 探究术前核磁共振成像 (MRI)在评估不同质地垂体瘤中的应用价 值。方法 收集2012年1月-2017年6月于 我院行手术治疗的98例垂体瘤患者的临 床病例、MRI影像学检查资料进行回顾性 分析,术前均行头颅MRI扫描,并根据 术中所见垂体瘤质地将其分为质地软组 (n=66)和质地韧组(n=32),使用天狼猩 染色检测肿瘤胶原含量。结果 两组性 别、肿瘤大小、功能类型及其生长方式的 差异均无统计学意义(P>0.05);两组在 T1WI瘤体与白质信号强度间比较差异无 统计学意义(P>0.05),质地软组T2WI瘤 体与白质信号强度比(2.13±0.58)%显著 强于质地韧组(1.38±0.42)%,差异有统 计学意义(t=6.525,P<0.05);质地软组 胶原含量(8.72±2.48)%显著低于质地韧 组(28.39±12.72)%,差异有统计学意义 (t=12.158,P<0.05);T2WI瘤体与白质 信号强度与胶原含量具明显负相关性(r=- 0.785,P<0.05)。结论 胶原沉积是垂体 瘤质地变坚韧的显著病理特征,可将T2WI 瘤体与白质信号强度比作为术前评估垂体 瘤质地的主要参考指标,指导手术入路的 选择。

Objective To explore the application value of preoperative magnetic resonance imaging (MRI) in the evaluation of different textures of pituitary tumors. Methods The clinical cases and MRI findings of 98 patients with pituitary tumors who underwent surgical treatment in our hospital from January 2012 to June 2017 were retrospectively analyzed. All patients were given cranial MRI scans before surgery, and they were divided into soft group (n=66) and firm group (n=32) according to the textures of pituitary tumors seen during operation. The collagen content in the tumor was detected by Sirius-Red staining. Results There was no significant difference in gender, tumor size, function type and growth patterns between the two groups (P>0.05). There was no significant difference in signal intensity ratio of T1WI tumor and white matter (P>0.05), and the signal intensity ratio of T2WI tumor and white matter in soft group was significantly higher than that in firm group [ (2.13±0.58)% vs (1.38±0.42)%] (t=6.525, P<0.05). The collagen content in soft group was significantly lower than that in firm group [(8.72±2.48)% vs (28.39±12.72)%] (t=12.158, P<0.05). The signal intensity ratio of T2WI tumor and white matter was negatively correlated with collagen content. (r=-0.785, P<0.05). Conclusion Collagen deposition is a significant pathological feature of pituitary tumor toughening. The signal intensity ratio of T2WI tumor and white matter can be used as the main reference index for preoperative evaluation of textures of pituitary tumors so as to guide the choice of surgical approach.

前言

垂体瘤系神经系统良性肿瘤,源于垂体前叶、后叶及颅咽管上皮 残余细胞,约占颅内肿瘤的5%~25%,可导致激素分泌异常,直接或间 接影响患者生长发育、工作学习及生育功能[1-2]。一般需行手术治疗, 可分为开颅手术及微创手术。随着近年来微创技术的不断发展,经蝶 窦入路手术凭借其手术创伤小、术后恢复快的优势成为临床首选疗 法[3]。垂体瘤的质地是决定手术入路选择的重要因素,采用经蝶窦入 路手术难以刮除质地坚韧的肿瘤,无法解除视路压迫,而采用经额或 翼点入路手术能尽可能地切除较多肿瘤,有效减轻视神经压迫。因此 术前评估垂体瘤质地对手术入路的选择及患者预后至关重要。多项研 究显示[4-5],核磁共振成像(MRI)除可作为垂体瘤鉴别诊断的依据外, 还可明确显示肿瘤大小、形态及与周围组织的关系。本研究旨在分析 不同质地垂体瘤患者的术前MRI影像学资料,试图为垂体瘤术前质地评 估及手术入路的选择提供依据,现报道如下。
