摘要
目的对比分析磁共振成像(MRI) 与多层螺旋CT(MSCT)在诊断2型糖尿病合 并急性胰腺炎中的应用。方法 选择医院 2014年2月-2018年2月收治的2型糖尿病合 并急性胰腺炎患者80例,所有患者均进行 MRI与MSCT检查,观察其胰腺改变、胰周 改变、肾前筋膜增厚及急性液体积聚情 况,并以病理结果为金标准,比较MRI与 MSCT诊断敏感度、特异性及准确度。结果 MRI检查的准确度为85.00%,MSCT检查的 准确度为86.25%,MRI与MSCT检查的准确 度差异无统计学意义(P>0.05)。MRI对胰 腺形态改变及胰周渗出、积液的敏感度高 于MSCT,差异有统计学意义(P<0.05)。 结论 MRI与MSCT在诊断2型糖尿病合并急 性胰腺炎中准确度相差不大,但MRI能敏 感地反映胰腺形态改变及胰周渗出、积液 情况。
Objective To compare and analyze the application of magnetic resonance imaging (MRI) and multi-slice spiral CT (MSCT) in the diagnosis of type 2 diabetes mellitus with acute pancreatitis. Methods A total of 80 cases of patients with type 2 diabetes mellitus complicated with acute pancreatitis admitted to the hospital from February 2014 to February 2018 were selected. All patients were given MRI and MSCT. The pancreas changes, peripancreatic changes, prerenal fascial thickening and acute fluid collection were observed, and the pathological results were taken as the gold standard to compare the sensitivity, specificity and accuracy of MRI and MSCT. Results There was no statistically significant difference in the accuracy of MRI and MSCT (85.00% vs 86.25%) (P>0.05). The sensitivity of MRI for changes of pancreatic morphology and exudation of peripancreatic effusion was higher than that of MSCT (P<0.05). Conclusion The accuracy of MRI and MSCT in the diagnosis of type 2 diabetes mellitus with acute pancreatitis are not much different, but MRI can better reflect the changes of pancreatic morphology and the exudation of peripancreatic effusion.

前言
急性胰腺炎是指多种病因引起的胰腺局部炎症反应,是一种极为严 重的急腹症。近年来,随着人们饮食结构的改变,急性胰腺炎的发病 率不断增加,2型糖尿病与急性胰腺炎关系密切。已有研究证实,糖尿 病并发急性胰腺炎的风险是普通人群的2-3倍[1-2]。另有有报道显示, 2型糖尿病是急性胰腺炎发病的危险因素之一[3]。影像学检查对于诊 断急性胰腺炎具有非常重要的作用,多层螺旋CT(MSCT)通过多层同层 技术,可迅速进行图像采集,在临床诊断急性胰腺炎中得到了广泛应 用,但其具有放射性,磁共振成像(MRI)检查对软组织具有较高的分辨 率,可清晰反映病变组织的具体情况,且对胰腺及胆道疾病的评估具 有较高诊断价值,在临床中被逐渐受到重视。MRI与MSCT虽广泛应用于 临床,但两者在2型糖尿病合并急性胰腺炎的应用比较研究还较少。本 研究通过分析对比MRI与MSCT的检查结果,旨在为临床诊断2型糖尿病 合并急性胰腺炎提供有效依据。现报道如下。
