18F-FDG PET-CT与胸部屏气螺旋CT联合在NSCLC术前TNM分期诊断中的应用*

摘要

目的研究18F-FDG PET-CT与 胸部屏气螺旋CT联合在非小细胞肺癌 (NSCLC)术前TNM分期诊断中的应用价 值。方法 将我院收治62例NSCLC患者纳 为研究对象,以病理检查结果作为“金 标准”,评估18F-FDG PET-CT与胸部屏 气螺旋CT在NSCLC术前分期中的应用价 值。结果 62例患者中,CT-T分期准确 性为80.65%(50/62),18F-FDG PET/CT-T 分期准确性为64.52%(40/62),两组T分 期准确率显著性差异(P<0.05);中央型 肺癌伴明显阻塞性改变患者CT-T分期准 确率为14.29%(1/7),18F-FDG PET/CT-T 分期准确率为71.43%(5/7),两组诊断正 确率差异显著(P<0.05);CT密度法-N分 期准确率为67.84%,摄取法-N分期准确 率为69.35%,CT密度+摄取法-N分期准 确率为77.42%,三组N分期准确率无显 著性差异(P>0.05);CT-M分期准确率为 91.94%,18F-FDG PET/CT-M分期准确率为 98.39%,两组M分期准确度无显著性差异 (P>0.05);CT-TNM分期准确性为80.65%, 18F-FDG PET/CT+CT-TNM分期准确性为 91.94%,两组TNM分期准确性无显著性差 异(P>0.05)。结论 CT联合18F-FDG PET/CT 可有效提高NSCLC患者术前分期准确率。

Objective To study the application value of 18F-FDG PET-CT combined with chest breath-hold spiral CT in the diagnosis of preoperative TNM staging of non-small cell lung cancer (NSCLC). Methods A total of 62 cases of patients with NSCLC admitted to our hospital were selected for the study. The results of pathological examination were used as the gold standard to evaluate the application value of 18F-FDG PET-CT and chest breath-hold spiral CT in preoperative staging of NSCLC. Results Among the 62 patients, the accuracy of CT-T staging was 80.65% (50/62), and the accuracy of 18F-FDG PET/ CT-T staging was 64.52% (40/62), and there was a significant difference in the accuracy of T staging between the two groups (P<0.05). The accuracy of CT-T staging was 14.29% (1/7) in patients with central lung cancer and obvious obstructive changes, and the accuracy of 18F-FDG PET/CT-T staging was 71.43% (5/7), and the difference in correct diagnosis rate was significant between the two groups (P<0.05). The accuracy of CT density-N staging was 67.84%, and the accuracy of uptake-N staging was 69.35%, and the accuracy of CT density and uptake-N staging was 77.42%, and there was no significant difference in the accuracy of N staging among the groups (P>0.05). The accuracy of CT-M staging was 91.94%, and the accuracy of 18F-FDG PET/CT-M staging was 98.39%, and there was no significant difference in the accuracy of M staging between the two groups (P>0.05). The accuracy of CT-TNM staging was 80.65%, and the accuracy of 18F-FDG PET/CT+CT-TNM staging was 91.94%, and there was no significant difference in accuracy of TNM staging between the two groups (P>0.05). Conclusion CT and 18F-FDG PET/CT can effectively improve the accuracy of preoperative staging of patients with NSCLC.

前言

肺癌已成为临床发病率、死亡率最高的恶性癌症之一,以非小细 胞癌(nonsmall cell lung cancer,NSCLC)为主,肺癌术前分期是医生 评估患者预后、制定治疗方案的有效依据[1]。CT是NSCLC常见的检查方 式,随着影像技术的不断发展,正电子发射计算接单层显像/计算机断 层成像(PET-CT)问世,该检查手段将PET与CT有效结合,实现了解剖 与功能的融合,在疾病诊断中具有良好的应用价值[2]。为研究CT、18FFDG PET-CT联合在NSCLC术前分期中的效果,我院开展如下研究。
