螺旋CT扫描及其多平面重建技术在腕舟骨骨折诊断中应用*

摘要

目的 探讨螺旋CT扫描及其多平 面重建技术(MPR)在腕舟骨骨折诊断中的 应用价值。方法 回顾性分析59例腕舟 骨骨折患者螺旋CT及X线片影像资料,比 较两种方式诊断腕舟骨骨折准确性,分 析螺旋CT及MPR诊断腕舟骨骨折分型情 况。结果 螺旋CT及MPR诊断腕舟骨骨折 的准确率为98.31%明显高于X线平片的 71.19%,差异有统计学意义(P<0.05); 螺旋CT及MPR诊断腕舟骨关节的分型显 示,5.17% A1型,13.79% A2型,5.17% B1型,43.11% B2性,20.69%B3型, 12.07% B4型。结论 螺旋CT及MPR可提高 腕舟骨骨折的诊断准确性,明确骨折分 型,可为确定治疗方案可供客观资料。

Objective To explore the application value of spiral CT scanning and multiplanar reconstruction (MPR) in the diagnosis of carpal scaphoid fracture. Methods The CT and X-ray imaging data of 59 patients with carpal scaphoid fracture were analyzed retrospectively. The accuracy in the diagnosis of carpal scaphoid fracture was compared between the two methods. The status of spiral CT and MPR in the diagnosis of carpal scaphoid fracture type was analyzed. Results The accuracy of spiral CT and MPR in the diagnosis of carpal scaphoid fracture (98.31%) was significantly higher than that of X-ray plain film (71.19%) (P<0.05). Spiral CT and MPR in the diagnosis of carpal scaphoid joint type showed that there were 5.17% A1 type, 13.79% A2 type, 5.17% B1 type, 43.11% B2 type, 20.69%B3 type and 12.07% B4 type. Conclusion Spiral CT and MPR can improve the accuracy in the diagnosis of carpal scaphoid fracture. They can determine the type of fracture and provide objective data for the determination of treatment regimen.

前言

腕舟骨骨折在腕骨骨折中较为常见,占全部腕骨骨折的50%~80% 左右,若未得到有效治疗,可能导致骨不连、腕关节疼痛等并发 症[1]。腕舟骨骨折位置特殊、结构复杂、血供特点等,X线征象有重叠 和失真,会增加漏诊和误诊几率,亦无法进行准确的临床分型,临床 应用中局限性。螺旋CT具有扫描速度快、图像分辨率的特点,加之强 大的后处理技术,可清楚显示骨折特征,便于临床早期诊治[2]。本文 回顾性分析59例腕舟骨骨折患者CT影像学资料,并与X线平片对比,探 讨螺旋CT及MPR在诊断腕舟骨骨折中的应用优势,现报道如下。
